Description

ChartSpan is the largest chronic care management (CCM) service provider in the US. CCM and Remote Patient Monitoring (RPM) programs focus on patients who have multiple (two or more) chronic conditions that are expected to last at least 12 months or more. We provide an essential service that helps providers stay in touch with and meet their patients’ healthcare needs in between office visits.

Position Overview:

The RN Escalation nurse serves as the senior clinical escalation resource for care management (CCM) and Remote Patient Monitoring (RPM) patient populations, assessing complex or urgent concerns escalated from the LPN Escalation team, inbound/outbound teams, and RPM device alerts.

Schedule:

This role is available on 1st shift, 2nd shift, and weekend schedules. Candidates will be assigned to a specific shift schedule upon hire, and coverage across shifts and weekends ensures continuous escalation support for patients.

This is a full-time, full-benefits remote role. All necessary equipment will be provided.

  • Responsibilities
  • Serve as the RN-level escalation resource for care management (CCM) and remote patient monitoring (RPM) patient populations.
  • Assess escalated patient concerns and RPM out-of-range readings/alerts, applying independent clinical judgment to determine urgency and appropriate intervention.
  • Conduct direct outreach to patients for urgent or complex clinical concerns identified through escalation or RPM monitoring.
  • Determine when a situation requires provider notification, emergency services referral, or immediate intervention, and act swiftly.
  • Communicate directly with ordering providers, specialists, and care teams to coordinate resolution of escalated concerns.
  • Provide clinical oversight and guidance to the LPN Escalation team on complex cases within RN scope of practice.
  • Thoroughly document all assessments, communications, and interventions in compliance with clinical and regulatory standards.
  • Identify trends in escalations or RPM alerts that may indicate the need for care plan revisions.
  • Maintain composure and sound decision-making during high-acuity or time-sensitive situations.
  • Provide consistent coverage according to assigned shift (1st shift, 2nd shift, or weekend).

Qualifications:

  • Current active, compact RN license.
  • Prior triage, telehealth, ED, urgent care, or similar experience preferred.
  • Strong independent clinical judgment and comfort making autonomous decisions in a fast-paced remote environment.
  • Excellent communication skills for coordinating with providers, patients, and internal clinical teams.
  • Ability to remain composed and effective during high-acuity or time-sensitive situations.
  • Willingness and flexibility to work an assigned shift, including 2nd shift or weekend coverage as needed.
  • Eagerness to assist fellow co-workers and contribute to team success.

Work Environment:

  • This position is fully remote. Equipment is provided.
  • This position operates in a professional office environment.
  • This position routinely uses standard office equipment such as computers and phones.
  • Scheduled hours may include evening or weekend coverage, depending on assigned shift.
  • Required Education:
  • RN degree (ADN or BSN) from an accredited nursing program.

Salary Description
$65,000 a year
