POSITION: LICENSED PRACTICE NURSE (LPN) – 

NURSE CLINIC (OUTPATIENT)

RESPONSIBLE TO: Nurse Manager

SALARY: Step Range: 20 ($51,244) - 30 ($68,868)

CLASSIFICATION: Professional, Regular, Full-time

LOCATION: Klamath Tribal Health & Family Services

(as assigned)

BACKGROUND: P.L. 101-630 level 

POSITION OBJECTIVES

The Licensed Practical Nurse (LPN) provides direct patient care in a nurse clinic within an outpatient setting. The LPN performs delegated nursing tasks and protocol-based care under the supervision of a Registered Nurse (RN) or licensed provider, in accordance with Oregon State Board of Nursing guidelines. Primary responsibilities include lab draws, immunizations, wound care, and other task-oriented clinical services.

MAJOR DUTIES AND RESPONSIBILITIES

1. Direct Clinical Care (Task-Based)

• Perform patient intake, including vital signs, allergies, and medication reconciliation
• Conduct venipuncture and capillary blood collection for ordered laboratory testing
• Process, label, and prepare specimens for transport in accordance with lab protocols
• Administer medications and injections, including vaccines, per provider orders and approved standing orders
• Monitor patients for immediate adverse reactions following medication or vaccine administration                                                       2. Nurse Clinic Services
• Provide immunizations in accordance with clinic protocols, standing orders, and state guidelines
• Perform wound care, including dressing changes, basic wound assessment, and documentation of healing progress
• Conduct ear lavage, simple treatments, and other approved clinical procedures
• Perform point-of-care testing (e.g., glucose, A1c, urine testing) per protocol
• Reinforce provider-directed patient education related to treatments, medications, and follow-up care                                              3. Scope of Practice Compliance (Oregon-Specific)
• Practice under the direction of an RN or licensed independent provider as required by the Oregon State Board of Nursing
• Perform only those duties that are within LPN scope and for which competency has been validated
• Follow established standing orders, clinical protocols, and delegated tasks
• Report patient status, abnormal findings, and concerns to the supervising RN or provider promptly
• Do not perform independent nursing assessments, triage, or care planning beyond LPN scope                                                          4. Documentation & Safety
• Accurately document all care, procedures, and patient interactions in the electronic health record (EHR)
• Maintain compliance with infection control standards, including proper handling of sharps and biohazard materials
• Ensure patient confidentiality in accordance with HIPAA regulations
• Follow medication administration safety standards, including the “Five Rights”                                                                                        5. Clinic Operations Support
• Maintain clean, stocked, and organized nurse clinic and treatment areas
• Ensure proper storage and handling of vaccines, including cold chain management
• Monitor and restock clinical supplies as needed
• Participate in competency validation and ongoing skills training                                                                                                                 6. Like all employees of the Klamath Tribes, the incumbent will be called upon to accomplish other tasks that may not be directly related to this position, but are integral to the Klamath Tribes’ broader functions, including but not limited to, assisting during Tribal sponsored cultural, traditional, or community events that enable the successful operation of programs and practices of The Klamath Tribes as aligned with The Klamath Tribes’ Mission Statement. Some of these tasks may be scheduled outside of regular work hours, if necessary.

KNOWLEDGE, SKILLS, ABILITIES

Ability to maintain strict confidentiality of medical records and adhere to the standards for health record-keeping, HIPAA and Privacy Act requirements.

Proficiency in venipuncture and specimen handling

Competence in vaccine administration and injection techniques

Knowledge of basic wound care and infection prevention practices

Understanding of LPN scope of practice and delegation requirements in Oregon

Ability to follow established protocols and standing orders with accuracy

Strong documentation and organizational skills

Effective communication with patients and healthcare team members

QUALIFICATIONS, EXPERIENCE, EDUCATION

Minimum Qualifications: Failure to comply with minimum position requirements may result in termination of employment.

• REQUIRED to possess a High School Diploma or Equivalent. (Must submit copy of diploma or transcripts with application.) 
• REQUIRED Current Licensed Practical Nurse (LPN) license in the state of Oregon
• REQUIRED Current Basic Life Support (BLS) certification
• REQUIRED Graduate of an accredited practical nursing program
• For out of state applicants; Oregon Licensure required within 90 days of hire. (Must submit copy of Licensure with application.) 
• REQUIRED to possess and maintain a valid Oregon Driver’s License, (out of state applicants must receive ODL within 90 days of hire), have good driving record and be insurable by the Klamath Tribal Health & Family Services’ vehicle insurance policy. (Must submit copy of driver license with application.)
• REQUIRED to submit to a background and character investigation, as per Tribal policy. Following hire must immediately report to Human Resource any citation, arrest, conviction for a misdemeanor or felony crime. 
• REQUIRED to submit to annual TB skin testing and adhere to KTHFS staff immunization policy in accordance with the Centers for Disease Control immunization recommendations for healthcare workers. 
• REQUIRED to acquire and maintain BLS certification within 180 days of hire; strongly recommend certification within 90 days of hire, probationary period will be extended for a maximum of 180 days pending certification. 
• REQUIRED to acquire and maintain ACLS certification within 180 days of hire; strongly recommend certification within 90 days of hire, probationary period will be extended for a maximum of 180 days pending certification. 
• REQUIRED to accept the responsibility of a mandatory reporter in accordance with the Klamath Tribes Juvenile Ordinance Title 2, Chapter 15.64 and General Council Resolution #2005 003, all Tribal staff are considered mandatory reporters.
• REQUIRED to be free from exclusion from providing Federal health care benefits including Medicare and Medicaid as per the Federal OIG and GSA exclusion lists. 

Preferred Qualifications:

• Previous experience in outpatient, primary care, or nurse clinic setting
• Experience with high-volume immunization clinics and lab draws
• Familiarity with EHR systems

Indian Preference:

• Indian and Tribal Preference will apply, as per policy. (Must submit tribal documentation with application to qualify for Indian Preference).
• Indian Preference will apply. In accordance with Klamath Tribal policy, priority in selection will be given to qualified applicants who present proof of eligibility for “Indian Preference”.
