Position Summary
East House is a dedicated non-profit organization committed to supporting individuals on their journey to recovery. We provide comprehensive services, including residential support, counseling, peer support and community outreach to empower individuals to overcome mental health and substance use disorders and lead fulfilling lives. The Bilingual (Spanish) Service Coordinator is responsible for partnering with clients in recovery from mental illness, chemical dependency, and/or co-occurring conditions to identify, access, and maintain linkages with treatment, housing, rehabilitation, and community services. Working within a person-centered approach, the Service Coordinator assists individuals in maintaining stable housing and improving their overall quality of life while teaching independent living skills.
Spanish language proficiency is an essential function of this position. Approximately 25% of the program's caseload consists of individuals whose primary language is Spanish. The ability to communicate directly in Spanish is essential to ensure accurate assessments, effective crisis intervention, meaningful home visits, service planning, funding coordination, and participation in social and community activities.
In compliance with NYS's Pay Transparency Act, the salary range for this position is listed above. RESPONSIBILITIES
- Partner with clients to develop individualized service plans based on client-identified goals, conducting assessments and service planning in English and Spanish to ensure clients fully understand their goals, services, and treatment options.
- Coordinate and advocate for access to mental health, substance use, physical health, financial, and other community resources, providing direct communication and advocacy in Spanish with clients, families, providers, and community partners as needed.
- Provide support in the community and in client homes, utilizing clinical knowledge and sound judgment.
- Participate in case reviews and planning to ensure quality and stability of client services.
- Support client access to housing, including landlord relations, lease retention, and financial assistance.
- Assist clients with benefit applications, budgeting, and payee agency coordination.
- Promote family well-being through school, recreation, and healthcare connections.
- Provide medication support as required by the program.
- Maintain documentation in accordance with funder and agency standards.

Working Conditions/Work Environment
Traditional office environment. Occasionally subjected to pressure due to time demands. Travel within the greater Rochester area. Potential for travel outside of Monroe County.

Desired Qualifications
Education
- High School Diploma or equivalent and 4-6 years of human services experience, OR
- Associate degree and 2-4 years of experience, OR
- Bachelor’s degree in human services and 1-3 years of experience.
- Experience
- Minimum of 1-3 years’ residential support or case management experience.
Skills/Abilities
- Professional fluency in spoken and written Spanish and English required.
- Demonstrated ability to conduct assessments, provide crisis intervention, and communicate complex behavioral health information in Spanish.
- Knowledge and experience supporting culturally diverse individuals with psychiatric or chemical dependency concerns.
- Skills in counseling, crisis assessment and intervention, communication, and client monitoring.
- Proficiency in Microsoft Word, basic computer literacy, and database usage.
- Licenses/Certifications
- Valid NYS driver’s license in good standing and access to a personal vehicle for work-related tasks.
