Central Baptist Village is seeking an experienced and compassionate Director of Social Services to lead our Social Services Department. This leadership role oversees the delivery of resident-centered psychosocial services across our Skilled Nursing, Assisted Living, Memory Care, and Short-Term Rehabilitation communities, ensuring residents and families receive compassionate support throughout their care journey.


The Director of Social Services provides department leadership, supervises and mentors team members, and collaborates with nursing, rehabilitation, medical providers, and interdisciplinary partners to promote quality outcomes and person-centered care. 

This role oversees psychosocial assessments, care planning, discharge planning, resident advocacy, regulatory compliance, quality improvement initiatives, and survey readiness.


The ideal candidate is passionate about supporting older adults, navigating complex psychosocial needs, developing strong teams, and making a meaningful impact within a mission-driven senior living organization.


Salary: $85,000–$90,000 annually


Responsibilities

  • Provide leadership and oversight for the Social Services Department and daily operations.
  • Supervise, mentor, and support Social Services team members while fostering professional growth and collaboration.
  • Oversee psychosocial assessments, individualized care planning, resident advocacy, counseling, and support for residents and families throughout admissions, transitions of care, and challenging life events.
  • Collaborate with interdisciplinary teams, including Nursing, Admissions, Therapy, Life Enrichment, and Administration, to support care planning, resident outcomes, QAPI initiatives, and regulatory survey readiness.
  • Assist residents and families with advance directives, guardianship, and connections to community resources.
  • Support residents experiencing behavioral, cognitive, and psychosocial challenges, including dementia-related needs.
  • Ensure compliance with CMS regulations, Illinois Department of Public Health requirements, and organizational standards through accurate documentation  and adherence to professional practices.
  • Develop and support programs that enhance resident quality of life and promote person-centered care.


Requirements
  • Master’s Degree in Social Work (MSW) from an accredited program.
  • Current Illinois Licensed Social Worker (LSW) license in good standing (LCSW preferred).
  • Minimum three (3) years of social work experience in long-term care, senior living, or a healthcare setting.
  • Minimum two (2) years of supervisory or leadership experience.
  • Experience working with older adults, including individuals living with Alzheimer’s disease and other dementias.
  • Knowledge of CMS and Illinois long-term care regulations.
  • Experience working in a CMS-regulated healthcare or long-term care environment preferred.
  • Strong interpersonal, leadership, documentation, and communication skills.
