Pay starts at $24 per hour or up based on experience in a group home setting.
Position Summary: The LPN at our CNR-OK group home would be responsible for the day-to-day medication pass and performing the functions of an LPN within the group home. These functions include but are not limited to taking and receiving verbal orders, educating patients regarding medications, documentation within the group home and community outings of the care of patients and participating in hospital and nursing programs as assigned.
Schedule: Monday - Friday, 8:00am - 5:00pm, some flexibility will be required based on need

Core Responsibilities:
- Complete or coordinate timely nursing assessments following accidents, injuries, medication variances, changes in condition, and other clinically significant events.
- Ensure required nursing documentation is accurate, complete, and entered as close to the time of the event as possible.
- Communicate resident needs, assessment findings, medication concerns, and changes in condition to providers and document all notifications and resulting orders or recommendations.
- Serve as the primary liaison between nursing and therapy to support continuity of care and prompt follow-up of resident needs.
- Attend clinical team and interdisciplinary treatment team meetings
- Support medication-related processes, including follow-up and documentation of medication variances, in accordance with hospital policy and scope of practice.
- Provide supplemental licensed nursing coverage for CNR outings and help coordinate clinical needs before, during, and after community activities.
- Participate in quality assurance activities by identifying documentation gaps, recurring clinical concerns, and opportunities for process improvement.
- Commuting patients to and from the main facility as well as community outings using company vehicle.

Qualifications:
- Valid license to practice nursing in the state of Oklahoma.
- Current AHA CPR certification required.
- Current CPI Certification.
- Current Food Handlers Permit.
- Must be able to lift/carry/push/pull 65lbs
- Must be 21 years of age
- Must have a clean driving record
- Group Home experience preferred but not required
