Halcyon Home is looking for a dependable, organized, and compassionate Care Coordinator / Overnight Receptionist to join our Home Care team in Austin, TX. This position is ideal for someone who enjoys working in a fast-paced environment, communicating with clients and caregivers, coordinating schedules, and ensuring the needs of our clients are met.
The successful candidate will be responsible for supporting daily operations, coordinating client care, maintaining accurate records, and serving as an important point of contact for clients, families, caregivers, and community partners. This is an overnight position and requires the ability to remain alert, responsive, and professional throughout the shift.

Key Responsibilities:
- Coordinate and document client schedules, schedule changes, incidents, and other client-related information accurately and in a timely manner.
- Coordinate care with management, caregivers, clients, families, and other members of the care team.
- Respond to client, family, caregiver, and agency needs in a professional and timely manner.
- Monitor staffing and scheduling needs and communicate concerns or changes to the appropriate team members.
- Conduct client file audits and identify and correct discrepancies.
- Maintain accurate electronic and paper records.
- Answer and manage a multi-line telephone system and appropriately direct calls and messages.
- Monitor agency communication systems, including phone, email, Teams, and other communication platforms.
- Receive, document, and communicate call-offs, schedule changes, incidents, and other important information.
- Provide clerical and administrative support, including data entry, filing, copying, preparation of packets, and special projects.
- Communicate effectively with clients, families, caregivers, community liaisons, physicians, and other healthcare professionals.
- Maintain strict confidentiality of client, employee, and agency information.
- Provide excellent customer service and represent Halcyon Home in a professional and positive manner.
- Support the agency's mission, values, policies, and commitment to quality care.
- Participate in required training, education, staff meetings, and in-services.
- Perform other duties as assigned to support clients and agency operations.
Schedule: Thursday, Friday, Saturday & every other Wednesday
Hours: 6:30PM – 8:30AM

Work Environment & Physical Requirements
- This position primarily works in an office environment but may occasionally require local travel. The role involves prolonged periods of sitting, with some standing, bending, reaching, and moving throughout the work environment.
- The employee may occasionally need to lift, carry, push, or pull items weighing up to 50 pounds.
- The position may involve working under stressful conditions, responding to urgent staffing or client needs, and meeting time-sensitive deadlines. Flexibility and the ability to work extended hours when necessary are important.
