YOUR TOP RESPONSIBILITIES
- Conduct comprehensive client assessments, develop individualized service plans, and support appropriate program placement.
- Maintain regular contact with clients, monitor progress toward goals, and adjust service plans as needs evolve.
- Coordinate support services and connect clients with medical, behavioral health, legal, employment, housing, educational, and community resources.
- Assist clients with applications for benefits and supportive services, including AHCCCS, SNAP, childcare assistance, and other available resources.
- Develop and maintain relationships with community partners and outside agencies to support client success and continuity of care.
- Maintain accurate and timely case documentation while ensuring compliance with organizational, licensing, funding, and confidentiality requirements.
- Schedule and coordinate client appointments, referrals, transportation, and follow-up services.
- Participate in case conferences and collaborate with multidisciplinary teams to ensure coordinated, client-centered care.
- Respond appropriately to client crises using trauma-informed practices, sound judgment, and de-escalation techniques.
- Transport clients to approved appointments and services as needed.
- Support clients in the self-administration of medications in accordance with physician orders, organizational policies, and regulatory requirements.
- Lead or participate in devotions, prayers, or other organizational activities that promote a credible Christian witness and lifestyle.
WHAT YOU SHOULD HAVE
- Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field preferred, or at least four years of experience providing case management or client services in behavioral health, recovery, social services, or a related setting; an equivalent combination of education and experience may be considered.
- Experience working with individuals experiencing homelessness, substance use disorders, mental illness, or co-occurring disorders preferred.
- Experience utilizing electronic case management systems and maintaining regulatory documentation preferred.
- Fluency speaking Spanish is preferred.
- Strong organizational, communication, and relationship-building skills.
- Proficiency with Microsoft Office and electronic case management or behavioral health documentation systems.
- Ability to exercise sound judgment, maintain professional boundaries, and respond effectively in crisis situations.
- A solid and credible Christian witness and lifestyle that exemplifies Christ in character and actions.
