Full -Time (hours may vary)

Pay Range: $16.21 - $24.48/hour


We are seeking a dedicated Wellness & Rehab Specialist to join our health and wellness team. This role offers an exciting opportunity to support individuals in their recovery and wellness journeys, promoting healthier lifestyles and improved quality of life. The ideal candidate will be passionate about helping others and possess a strong background in rehabilitation and wellness practices.


Minimum Qualifications:

  • High school diploma or equivalent.
  • At least one year of medical-related experience in a long-term care, senior living, rehabilitation, or hospital setting, preferably in a Rehabilitation Technician or similar role.
  • Strong verbal and written communication skills.
  • Ability to work effectively and collaboratively as part of an interdisciplinary team.
  • Reliable, responsible, and able to exercise sound judgment and initiative.
  • Ability to follow verbal and written instructions.
  • Strong organizational, time-management, and documentation skills.
  • Ability to interact respectfully and effectively with older adults, including individuals living with dementia.
  • Ability to obtain required dementia-related training/certification, including Montessori and Snoezelen training, as required by King’s Grant.
  • Ability to obtain required CPR Instructor certification through the American Heart Association/SOVAH training program, with training provided by King’s Grant as applicable.

Preferred Qualifications:

  • Current CNA certification in addition to required rehabilitation experience.
  • Experience working in long-term care, assisted living, memory care, rehabilitation, or senior living.
  • Knowledge of restorative nursing principles and practices.
  • Understanding of the restorative care philosophy and approaches that promote resident independence.
  • Experience working with individuals living with dementia.
  • Experience leading wellness, fitness, rehabilitation, or resident engagement programs.
  • Experience with electronic medical records and Microsoft Office applications.
  • Experience with group education, public speaking, or community wellness programming.
Requirements

Wellness Program

  • Develop, implement, and support comprehensive wellness programs that incorporate principles and resources from Therapy, Nursing, Restorative Care, and Life Enrichment.
  • Promote a culture of health, wellness, independence, and resident engagement throughout the campus.
  • Conduct community engagement activities designed to promote wellness initiatives and increase resident participation.
  • Provide resident-centered support using motivational interviewing and other appropriate communication techniques to encourage healthy lifestyle choices and behavior changes.
  • Gather resident life stories, personal histories, interests, and dementia life stories to help support individualized wellness and engagement.
  • Collaborate with healthcare providers, therapy professionals, community organizations, and referral sources to coordinate services and facilitate access to appropriate resources.
  • Assist with outreach and distribution of marketing materials to community referral sources regarding services such as Outpatient Therapy, Lymphedema Therapy, and Aquatic Therapy.
  • Distribute Welcome Wellness/Rehab Kits to new residents upon admission, as appropriate.
  • Develop and deliver engaging educational presentations on health and wellness topics, including outpatient therapy services, lymphedema, aquatic therapy, multisensory programming, body mechanics, stretching, range of motion, and other relevant topics.
  • Assist residents with range-of-motion exercises and stretching during aquatic sessions, as appropriate and within the scope of the position.
  • Collect, maintain, and analyze program data related to participation, effectiveness, resident progress, and community needs.
  • Utilize the EMR and other approved systems to document program participation, resident interactions, progress, and other required information.
  • Maintain accurate and timely documentation of resident interactions, program activities, and health-related information.
  • Assist with the development and implementation of new wellness initiatives and programs as directed.
  • Assist with special wellness programs, such as multisensory or other therapeutic wellness initiatives, including participant scheduling, room preparation, set-up and clean-up, and facilitation as assigned.

Restorative Program

  • Provide restorative programs and interventions under the direction of licensed therapy professionals.
  • Assist residents with mobility, ambulation, gait training, strengthening, activities of daily living, feeding, positioning, range of motion, contracture management, and other approved restorative programs.
  • Support residents in maintaining their highest practicable level of independence and functional ability.
  • Monitor functional maintenance programs completed by CNA staff and communicate concerns or changes in resident function to therapy and nursing staff.
  • Observe and provide education regarding safe transfer techniques, gait belts, mobility, positioning, and other appropriate safety practices.
  • Recognize and promptly report changes, declines, or concerns related to a resident's functional status.
  • Accurately document resident encounters, interventions, participation, and services provided.
  • Participate in interdisciplinary communication and care coordination related to resident function and restorative needs.
  • Conduct or participate in therapy/nursing team huddles related to post-fall management, resident safety, mobility, and education.
  • Maintain a clean, organized, and sanitary work environment.
  • Promote resident safety, dignity, quality care, and a positive resident experience at all times.

Nursing Support

  • Provide supplemental support to Nursing during periods of increased resident care needs or high-volume activity.
  • Assist with resident feeding, as assigned and appropriate.
  • Assist Nursing with toileting rounds, particularly during high-volume periods such as mealtimes.
  • Assist with positioning residents during wound rounds under the direction of nursing staff.
  • Communicate observations or concerns regarding resident condition, safety, or care needs to appropriate nursing personnel.
  • Participate as a co-instructor for monthly CPR training sessions after completing required instructor certification and training.

Life Enrichment and Activities

  • Conduct and support community engagement activities that promote wellness, socialization, participation, and quality of life.
  • Assist with large monthly campus events, including decorating, transporting residents to and from events, assisting residents during events, and providing other support as needed.
  • Accompany Activity Coordinators on resident outings and provide appropriate assistance with activities of daily living and resident needs during transportation and events.
  • Assist with Life Enrichment programs that support resident engagement, socialization, cognitive stimulation, and emotional well-being.
  • Participate in grant-related projects and assist with data collection and documentation.
  • Assist with the development of resident life-story materials and projects, including 2Nomi Life Story scrapbooks, as assigned.
  • Conduct engagement sessions in the Multisensory Room to support resident quality of life and individualized therapeutic goals.
  • Maintain accurate documentation of multisensory room sessions, resident participation, responses, and other required information.
  • Participate as a co-facilitator in the monthly Dementia Support Group.
  • Assist with dementia-related education and grant initiatives, including Reality Vision Dementia sessions.
  • Provide supplemental staffing support in Memory Care when needed to maintain the required minimum staffing level, including coverage when regular Memory Care staff are on breaks or temporarily away from the unit.
  • Promote a safe, supportive, engaging, and person-centered environment for residents living with dementia.

Documentation and Communication

  • Maintain timely, accurate, and complete documentation in the EMR and other designated systems.
  • Document resident participation, interventions, progress, responses, and significant observations according to established procedures.
  • Communicate changes in resident condition, function, behavior, participation, or safety to the appropriate department or licensed professional.
  • Protect resident confidentiality and maintain compliance with HIPAA and King’s Grant policies and procedures.
  • Participate in interdisciplinary meetings, huddles, education sessions, and other meetings as assigned.
  • Maintain effective communication with residents, families, staff members, healthcare professionals, and community partners.

Resident Safety and Customer Service

  • Provide compassionate, respectful, and resident-centered service.
  • Maintain resident dignity, privacy, independence, and choice.
  • Follow established infection prevention, safety, emergency response, and infection-control procedures.
  • Recognize and report safety concerns, accidents, injuries, changes in condition, or other unusual occurrences promptly.
  • Demonstrate professionalism and maintain appropriate boundaries when interacting with residents, families, visitors, and coworkers.
  • Contribute to a positive, collaborative, and team-oriented work environment.
  • Other duties as assigned.
