Summary:
Care Coordinator in the medication room coordinates client appointments, performs medication reconciliation, and supervises client self-administration of medication following all regulatory requirements. Working collaboratively with the nurses, the care coordinator ensures best client care for the clients at the 56 Recovery Programs.
Essential Duties & Responsibilities:
- Supervises client self-administration of medication following all regulatory requirements.
- Guide, train, and mentor staff on proper med room protocols, safety standards, and daily operational workflows.
- Act as the primary administrative and operational bridge for the nursing team, streamlining daily medication workflows, shift-change handoffs, and administrative communication.
- Maintain complete safety, regulatory compliance, and organization within the medication room. Conduct routine, scheduled audits of medication supplies, expiration dates, secure storage conditions, and controlled substance logs.
- Participate in multidisciplinary discharge or placement meetings to ensure the client's medication supply chain is fully established prior to arrival or discharge.
- Performs treatment planning related tasks including coordinating appointments and placements for all clients.
- Effectively responds to and takes a lead in all medical emergencies on the unit.
- Documents in the clinical record according to requirements of the regulatory agencies.
- Ensures that the medication room is clean and organized at all times and that all medications are appropriately stored, labeled, and documented in the client Medication Administration Record.
- Effectively performs client medication reconciliation at admission and after each medication change, as directed by the medical provider.
- Responsible for coordinating all client medication needs, including obtaining prescriptions, refills, and discontinuing medication and preparing consultation reports for all appointments and Emergency Department.
- Facilitates groups as required.
- Required to transport clients in personal or Agency vehicle
- All other duties as assigned.
*All agency staff are required to attend all mandatory department/agency meetings and trainings
Schedule:
Sunday, 7:00 AM - 3:30 PM, Monday - Thursday, 10:00 AM - 6:30 PM
Education &/Or Experience:
- Experience supervising the administration of medication is required.
- Pharmacy Technician, Medical Assistant or Certified Nursing Assistant.
