HealthFirst Family Care Center is seeking a compassionate, collaborative Primary Care Physician to join our team and provide high-quality, patient-centered care to individuals and families in the communities we serve.

As a Federally Qualified Health Center, HealthFirst provides comprehensive primary care to patients throughout the Lakes Region of New Hampshire. Our providers care for a diverse patient population and work alongside an experienced, multidisciplinary team committed to improving health, removing barriers to care, and building lasting relationships with our patients.


What You'll Do

As a Primary Care Physician at HealthFirst, you will:

  • Provide comprehensive primary care focused on prevention, wellness, and management of acute and chronic conditions
  • Develop individualized treatment plans based on each patient's medical and psychosocial needs
  • Educate and counsel patients to help them actively participate in their health and wellness
  • Collaborate with physicians, advanced practice providers, nursing staff, behavioral health professionals, and other members of the care team
  • Participate in quality improvement and population health initiatives
  • Serve as a clinical resource and support to members of the care team
  • Maintain timely, accurate documentation in the electronic health record
  • Participate in the clinic's on-call rotation as applicable

What We're Looking For

  • MD or DO from an accredited medical school
  • Completion of an accredited residency in Family Medicine or Internal Medicine
  • Board certification
  • Current or eligible New Hampshire medical license
  • Current DEA registration
  • Strong communication and collaboration skills
  • A commitment to compassionate, respectful, patient-centered care

Requirements

EDUCATION/EXPERIENCE:

Graduate of an accredited Medical School and completion of accredited medical residency in either Family Practice or Internal Medicine. New Hampshire license, Board certified with current DEA.


KNOWLEDGE, SKILLS, and ABILITIES:

  • Demonstrates effective communication skills, ability to interact within a team setting, and demonstrate professional behavior and appearance.
  • Displays a courteous and supportive manner with staff as well as a competent, confidential, and compassionate manner with patients.
  • Ability to assist clinical staff in interpretation of medical charts, monitors, and other pertinent patient medical data.
  • Computer skills and documentation in electronic medical records is necessary.
  • Collaborates with members of other disciplines, community agencies, school personnel, and hospital staff in meetings the needs of the community.
  • Acts as a consultant, resource and support for the clinical staff.
