Host Family Respite Worker (Part Time - Flexible Hours)

Description

This position will evaluate and provide training to the members in daily living, social skills to assist them in achieving and maintaining their maximum level of functioning with independent community living. Oversite of host caregiver responsibilities and adherence to required tasks. Responsible for case management, and development of the Host Family Homes, located on the South/North side and Suburbs of Chicago. 


Essential Responsibilities:

· Oversee host family caregivers, including monitoring the Host Family homes monthly, by completing monthly visits twice a month. Coordinate with the Community Support Team as needed per caseload.

· Monitor and assists in maintenance of documentation, current records in the homes, such as medication records, goal progress and fire/safety drills. Complete case notes after home visits, in a timely manner.

· Assist the members in achieving their maximum level of independent functioning regarding goals as stated in the Person-Centered Plans. Ensure that the members integrate and spend time in the community.

· Implement group or individual programs which promote growth and development toward independent living skills for members.

· Complete and ensure the timeliness of all assigned documents and forms to include daily progress reports, incident reports, MAR sheets, etc.

· Monitor the Host Family Homes to ensure they are kept safe, clean and provide healthy environment for the members. Function as an advocate for the members to help them reach their full potential.

· Monitor the members to ensure the safety, and cleanliness of the caregiver’s home, including checking the water temperatures.

· Supervise and assist the members if needed, in taking their medication in accordance with program guidelines, maintaining confidentiality and respecting rights of the members.

· Attend and participate in all mandatory meetings/staffing’s and training as required by the agency and Supervisor.

· Respond to medical and behavioral emergencies following appropriate Health and Safety, First Aid, CPR, and other procedures, as necessary.

· Immediately report any medical/behavioral issues, emergencies, or other unusual incidents to Host Family Coordinator and/or Supervisor.


Other Responsibilities:

Assist to transport and/or accompany the HF members to appointments and other services, including work centers, as needed.

Observe the member’s rooms with their permission during home visits to ensure their wellness and safety.

Check and ensure all doors, windows are working properly in the home to adhere to building safety and adherence to the use of universal precautions in accordance with agency policies and regulatory guidelines.

Maintain all required certifications and trainings and keep such certifications current. Participate in supervision meetings with direct supervisor at least once a month; and perform other related duties as assigned.

Requirements

Two years of full-time related work experience or two years of college or six years’ experience working as a Direct Support Professional experience. 

Must have proof of Direct Service Professional (DSP) training and pass Literacy test within 120 days of hire. 

Must have a valid driver’s license, current vehicle insurance and access to a reliable vehicle for transport between caregiver homes and to transport members to appointments, if needed. 

Must be pass required training to become authorized to pass medications, as needed.
