Pay: dependent on Case Manager certification status

  • Uncertified starts at $17.50 — Certification will be obtained within the first 120 days and pay will be increased to the certified rate.
  • The certified range starts at $19.61 — pay is calculated based on years of certified experience. 

Shift Differential: Additional  $2.00 /hour when clocked in for 3+ hours between 3:00pm - 11:00pm


Schedule: Tuesday - Saturday | 4:00pm - 12:00am (Midnight)

Job Summary

Case Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals. Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.


Essential Functions

  • Provides psychosocial rehabilitative services as described by the Medicaid manual
  • Supports clients in following individualized care plans and identifying case management goals
  • Completes documentation on services provided within Valley’s standards
  • Meets productivity expectations as assigned by supervisor
  • Distributes medications as necessary
  • Transports clients to appointments as necessary
Requirements

Education

  • High School diploma or equivalent

Experience

  • None

Licenses/Certificates

  • Case Manager certification
  • CPR certification (Post-hire)
  • Valley de-escalation certification (Post-hire)
  • Driving positions require a minimum age of 21 and a current driver’s license
