JOB TITLE: Case Manager/QIDP
DEPARTMENT: Residential Services
SUPERVISOR’S TITLE: Residential SGL Director
JOB STATUS: Full-time ; Non-Exempt

SCHEDULE: Monday - Friday (8am-4pm)


Summary:

The Case Manager/QIDP coordinates person-centered services for individuals receiving residential supports. This role develops, monitors, and updates service plans; advocates for residents; and works with staff, families, service providers, and state agencies to ensure quality, compliant services.


Key Responsibilities:

  • Lead annual assessments and interdisciplinary team meetings for each resident’s Person-Centered Plan.
  • Develop, update, distribute, and maintain Individual Service Plans and required resident documentation.
  • Monitor resident progress, complete quarterly evaluations, and revise plans as needs or goals change.
  • Coordinate with Home Managers and staff to ensure service plans are implemented correctly.
  • Advocate for residents’ rights, preferences, goals, and community participation.
  • Collaborate with residential, vocational, day program, therapy, family, guardian, and agency partners.
  • Maintain resident records, confidentiality, required reports, and state-agency documentation.
  • Assist with corrective action plans and compliance requirements from the Indiana Department of Health and other governing agencies.
  • Provide on-call crisis intervention and resident support as needed.
  • Assist residents with benefit applications, financial resources, and required agency notifications.
  • Participate in required training and perform other assigned duties.

Qualifications & Skills:

  • Strong verbal and written communication skills.
  • Ability to build professional relationships with residents, families, coworkers, service providers, and the public.
  • Strong organization, documentation, and time-management skills.
  • Ability to work independently and collaboratively as part of an interdisciplinary team.
  • Proficiency with word processing, database, and spreadsheet software.

Certifications & Training Requirements:

  • Maintain current CPR, First Aid, and Preventing Disease Transmission certification.
  • Maintain current CPI/Non-violent Crisis Intervention certification.
  • Maintain a valid driver’s license, vehicle liability insurance, and reliable transportation.

Requirements

 Education Requirements:

  • Bachelor’s degree in a human services field required.
  • Must meet the qualifications to serve as a Qualified Intellectual Disabilities Professional (QIDP).

Experience Requirements:

  • At least one year of experience working with individuals with developmental disabilities required.
Salary Description
$17-$20
