JOB SUMMARY:
The Community Outreach Representative is primarily responsible for providing the most up-to-date education and information on various services and programs to healthcare professionals like physicians, caregivers and the general community. Strengthening relationships to ensure referrals, visibility, and program awareness is also an important responsibility of this position.
EDUCATION, CREDENTIALS & EXPERIENCE REQUIREMENTS:
- A degree in Nursing or a Related Healthcare Field is required, though prior experience in healthcare marketing is a definite advantage.
- 1-5 years healthcare experience, and some experience assisting in fundraising preferred
- Excellent oral and written communication
- Ability to build and maintain positive relationships
- Competency with equipment and medical information pertaining to various chronic diseases, particularly those treatable with outpatient IV administration.

WORKING CONDITIONS:

JOB SPECIFIC COMPETENCIES:
- Markets and promotes services of healthcare organization and programs on a scheduled basis.
- Manages and coordinates outreach events, health screenings, and other requests for organizations, businesses and municipalities in order to attract patients.
- Strengthens existing relationships and cultivates new ones through working with physician practices and new referring physicians alike.
- Strategically reaches out to facilities for home care services, difficult to place patient, referrals and other marketing opportunities.
- Coordinate professional presentations to community groups on educational topics (i.e. vent/dialysis patients)
- Maintain and execute the established business development and marketing plan through strategic relationship-building, physician outreach, referral source management, specialty service line promotion, calendar management, performance tracking, and community engagement activities, resulting in increased referrals, patient volumes, fundraising opportunities, and overall organizational growth
- Maintains a structured and organized outreach calendar, scheduling regular visits, meetings, educational presentations, physician-office appointments, community events, and networking opportunities to maximize visibility and referral development.
- Utilizes CRM, spreadsheets, or other tracking tools to document outreach activities, referral sources, provider interactions, marketing initiatives, and follow-up actions.
- Monitors and analyzes key performance metrics including referral volumes, conversion rates, provider engagement, patient volumes, outreach effectiveness, and service line growth.
- Develops monthly and quarterly reports outlining marketing activities, referral trends, return on outreach efforts, and strategic growth opportunities.
- Establishes measurable goals and benchmarks for specialty service lines including Endocrinology, Allergy/Immunology, Rheumatology, Infectious Disease, Infusion Therapy, and Home Health services.
- Evaluates the success of marketing campaigns, physician outreach efforts, community education programs, and special events, making recommendations for continuous improvement.
- Collaborates with leadership to identify growth opportunities and implement strategies that support increased referrals, patient access, service utilization, and community awareness.
- Maintains accurate records of referral patterns and provider engagement activities while ensuring timely follow-up with referral sources and prospective community partners.
