Heartline Staffing is recruiting a Primary Care Physician for an onsite opportunity in Plantation, Florida, providing comprehensive, value-based primary care to senior patients. Spanish-speaking ability is preferred for this location but is not required. This physician will manage a panel of up to 450 patients while leading a dedicated care team and maintaining accountability for patient outcomes and the detailed care of each patient.
POSITION OVERVIEW
This is an outcomes-focused primary care model centered on strong physician-patient relationships, health coaching, care coordination, and physician leadership. The PCP independently manages acute and complex chronic conditions common in older adults while working collaboratively across the care continuum.

COMPENSATION
• Base Salary Range: $221,141-$315,915
• Position May Also Be Eligible For Bonuses Or Commissions
ROLE
• Provide Comprehensive Care For Older Adult Patients With Acute And Chronic Illnesses
• Assess, Diagnose, Treat And Manage Acute And Complex Chronic Health Needs
• Maintain Accountability For Patient Care And Outcomes
• Build Trusting Relationships With Patients And Provide Ongoing Health Coaching
• Help Patients Establish And Work Toward Short-Term And Long-Term Health Goals
• Provide Patient Education, Health Promotion, Care Coordination And Documentation
• Lead A Dedicated Care Team
• Collaborate With Specialists While Remaining Involved In The Detailed Care Of The Patient
• Engage With Hospitalists When Patients Are Hospitalized
• Coordinate With Transitional Care Teams For Patients In Hospitals, Skilled Nursing Facilities, Long-Term Care Or Homebound Settings
• Collaborate With Case Managers, Acute And Transitional-Care Physicians And Other Available Resources
• Participate In Risk Management, Quality Management And Clinical Meetings
• Help Cover Other Providers As Needed
• Participate In Center Management And Support A Positive Center Culture
• Supervise Or Collaborate With Advanced Practice Practitioners As Applicable
• Work Toward Clinical, Patient Experience, Quality And Medical Cost Performance Goals
TRAVEL / FLEXIBILITY
• Local Travel Within The Market As Needed, If Applicable
• National Travel For Initial 2-Week Training
• Occasional Regional And National Travel For Recruiting Or Training Purposes
• Flexibility To Work Evenings, Weekends And / Or Holidays As Needed
REQUIREMENTS
• MD Or DO In Internal Medicine, Family Medicine, Geriatrics Or Similar Specialty REQUIRED
• Ability To Obtain A Florida Medical License Or Current Active Florida Medical License REQUIRED
• Board Eligible In Internal Medicine, Family Medicine, Geriatrics Or Similar Specialty REQUIRED
• Current DEA Number For Schedule II-V Controlled Substances REQUIRED
• American Heart Association Or American Red Cross BLS Certification Within The First 90 Days Of Employment REQUIRED
• Fluency In Written And Verbal English REQUIRED
• Ability And Willingness To Complete Required Travel, Including National Travel For Initial 2-Week Training REQUIRED
• Flexibility To Work Evenings, Weekends And / Or Holidays As Needed REQUIRED
PREFERRED
• Spanish-Speaking Ability
• Board Certification In Internal Medicine, Family Medicine, Geriatrics Or Similar Specialty
