Description
Position Purpose:
To provide Direct Resident Care under supervision of the Charge Nurse. The Nurse Assistant follows established policies and procedures while providing and coordinating resident care with other facility personnel.
Performance Standards:
- Follow all facility policies, procedures, performance standards, and applicable federal, state, and local regulations.
- Seek clarification or additional training when policies or procedures are not understood and assist new employees as appropriate.
- Provide resident care safely, respectfully, and in accordance with Residents' Rights, assigned care plans, and nursing direction.
- Complete resident care assignments, rounds, documentation, and required records accurately and timely.
- Observe, report, and document changes in resident condition, incidents, unusual occurrences, safety hazards, and resident concerns promptly.
- Participate in care conferences, education programs, in-service training, and assigned meetings.
- Demonstrate competence in required nursing assistant skills and seek assistance when resident needs require additional assessment.
- Follow facility procedures for admissions, transfers, discharges, equipment, and supply management.
- Maintain positive, professional relationships with residents, families, physicians, coworkers, and other healthcare team members.
- Respond promptly to call lights and resident needs, demonstrating compassion, patience, and respect at all times.
- Support resident safety, comfort, dignity, and overall well-being through attentive and responsive care.
- Work cooperatively with all departments and maintain a positive, team-oriented attitude.
- Maintain regular attendance, punctuality, professional appearance, and adherence to the facility dress code.
Job Tasks:
- Clock in, receive report, and review assignments.
- Provide daily care to assigned residents.
- Assist with meals, feeding, hydration, and nourishment.
- Assist residents with toileting, bathing, grooming, oral hygiene, and dressing.
- Change incontinent residents and maintain resident comfort and dignity.
- Transfer, ambulate, reposition, and transport residents as needed.
- Ensure resident safety by keeping call lights accessible and following bed rail and positioning protocols.
- Monitor residents and promptly report any changes or concerns to the Charge Nurse.
- Keep resident rooms, bedside tables, equipment, and care areas clean and organized.
- Assist residents in attending activities and dining room meals.
- Maintain accurate records, including intake/output, bowel and bladder records, food consumption, and other required documentation.
- Empty and record urinary drainage units as assigned.
- Verify resident identification bands and report discrepancies.
- Complete charting and resident progress notes before the end of the shift.
- Provide shift report to oncoming staff.
- Perform other duties as assigned by the Charge Nurse, Supervisor, or Director of Nursing Services.

Requirements

- CNA licensed by State of CA - Required
- CPR & First-Aid Certification - Required
- Fire Card (Hospital Fire & Life Safety)
- Must clear criminal background with DOJ and FBI
- Ability to work in a healthcare setting with the senior population
Physical Requirements of Position
- Ability to lift fifty (50) pounds; Moving, lifting, assisting or transferring of patients may involve lifting of up to 100 pounds using proper mechanics and/or assistive lifting devices.
- Ability to exert maximum muscle force to lift, push, pull or carry patients
- Ability to use abdominal and lower back muscles over time without fatigue
- Ability to stand for extended periods
- Ability to bend, stoop, stretch, twist, sit, and reach
- Fine motor skills
- Visual acuity
