What You'll Do As A Hospice Social Worker

  • Support patients and families through the emotional and practical challenges of serious illness.
  • Provide counseling, care planning, and resource coordination.
  • Help families navigate advance directives, care transitions, and end-of-life decisions.
  • Facilitate grief and bereavement support services.
  • Collaborate with an interdisciplinary hospice team to deliver compassionate, patient-centered care.
  • Document services and maintain compliance with hospice standards.
  • Participate in on-call coverage as scheduled.

Qualifications

  • Master's degree in Social Work (MSW) preferred; Bachelor's degree in Social Work, Psychology, Sociology, or related field required. 
  • LCSW licensure preferred. 
  • Minimum of two years of social work experience, including at least one year in a healthcare setting. 
  • Clinical experience working with individuals, families, and groups.
  • Driver’s license, personal automobile and current automobile liability insurance
