Reports To: Director of Social Services
Status: 80 hours per two week pay period
Shift: 8:00 am – 4:30 pm
Job Schedule: Full time
Weekly Hours: 40 hours

HRMC is looking for applicants to help bridge the gap between the community and health and social service system by providing one on one support to individuals and families. Enhance communication between community members , healthcare resources , organizations, and businesses through informal education, prescription assistance and assist in navigating through the health care system. This position will maintain and practice cultural competency and integrity within the community.

- Certification or willing to obtain Community Healthcare Worker from an accredited college
- Must have a valid drivers license and reliable personal transportation
- Familiar with the community resources, 211 program, and local healthcare services
- Excellent written, verbal, and non-verbal communication skills
Working Conditions:
- Work indoors will be in clean, well-lighted, climate-controlled environment with minimal hazards under normal conditions.
- Work will require travel to patient homes which could expose you to a variety of settings.
- Work requires finger/hand dexterity and eye/hand coordination along with accurate keyboarding skills.
- Position involves sitting, standing, keyboarding, filing, occasional light lifting, walking, stooping, squatting, and bending.
- Risk exposure to communicable disease.
