Description

Job Summary: The Home Care Aide is responsible for providing personal care services according to written directions prepared by the R.N. and carried out under the supervision of the appropriate professional.


Demonstrates Competency in the Following Areas:

· Demonstrates ability to assist patients with activities of daily living, e.g. personal hygiene, exercises, ambulation, and meal preparation.

· Provides household duties (cooking, light housework, linen changes, and laundry) to facilitate the patients care at home.

· Demonstrates basic positioning of bedbound patient.

· Demonstrates correct Range of Motion on assigned patients per plan of care.

· Demonstrates ability to administer patient care accurately as per plan of care.

· Supports the patient and provides appropriate setting to encourage independence.

· Demonstrates ability to make home visits to patients as assigned at appropriate frequency.

· Demonstrates ability to assist with implementation of emergency actions as per home care policy.

· Demonstrates ability to appropriately set priorities and is able to carry out work accurately and efficiently.

· Demonstrates ability to take vital signs accurately as ordered on patients and documents legibly and accurately on visit records.

· Demonstrates ability to assist patients with assistive devices as per plan of care.

· Demonstrates ability to complete visit record accurately and legibly, using appropriate language, and returns in a timely manner.

· Completes daily clinical logs accurately and returns in a timely manner.

· Communicates effectively with patients, families, significant others, and other staff members.

· Gives factual information about assigned patients to Case Manager or Director.

· Demonstrates ability to identify and report any obvious patient problems or changes in condition to the R.N. case Manager.

· Demonstrates knowledge of the principles of growth and development over the lifespan.

· Contributes to the accuracy of patient charges and prevents waste of hospital/patient resources.

· Reports to work on time and as scheduled and completes work within designated time.

· Avoids excessive and/or unauthorized overtime.

· Adheres to JCMH policies for requested and/or emergency time off.

· Adheres to dress code and is professional in appearance and conduct.

· Supports necessary changes made and is not detrimental to the harmony of the department.

· Maintains regulatory requirements.

· Completes annual educational requirements.

· Uses time card accurately.

· Communicates the mission, ethics, and goals of the Hospital as well as the mission and vision of Home Care.
