Wheatridge Park Care Center is seeking a C.N.A. Resident Scheduling/Transportation Coordinator to join our team!
In your role as the C.N.A. Scheduling/Transportation Coordinator, you will assist in coordinating facility staffing/transportation for our community. Assist residents’ transportation needs.

Transportation/Scheduling Responsibilities:
- Coordinate resident appointments.
- Transports Community residents to and from medical appointments, new admissions, activities, and shopping errands using Community-owned vehicle.
- Maintain current and up to date records of resident appointments.
- Coordinate and communicate appointments with nursing department and Leadership
- Works with other transport companies to ensure appropriate transportation is available
- Successfully meets all requirements of Transportation Driver position, including Driver Record report; meets and completes ongoing training and driver requirements.
- Reads the transportation schedule daily and makes sure all scheduled appointments can be met.
- Is flexible for unscheduled transportation needs in an emergency.
- Revises schedule accordingly in order to stay with and assist residents who are unable to access community and appointments independently.
Requirements
Education Requirements:
- Must have current C.N.A. License
- Must have current driver's license and a good driving record.
- High school diploma or GED preferred.
- Successful completion of CPR/First Aid Course, Driver Safety and Training Course preferred.
- Previous experience in a driving position preferred.
Physical Requirements: Transportation coordinators understand the physical demands their jobs require and realize they need to:
- pass a physical exam.
- lift, push, pull and move equipment, supplies, etc.
- lift a maximum of fifty (50) pounds occasionally.
- lift twenty-five (25) pounds consistently.
- be able to move and stretch in such a way as to be able to reach areas above their head and below their waist.
- go up and down stairs.
- bend, stoop and twist almost constantly throughout the day.
- see and hear well enough to answer phones, greet people and read
- remain active and on their feet for up to four (4) hours at a time.
