About the Role

Sierra Health Care has been the home health provider Las Cruces families have trusted since 1982. As Clinical Manager for Home Health, you'll lead the clinical team that makes that trust possible — overseeing care delivery, supporting staff, and ensuring every patient receives safe, effective, and compassionate care. This is a leadership role for a clinician who is ready to step back from the bedside and invest in the team and systems that make great care happen.


Key Responsibilities

  • Supervise, orient, and conduct ongoing competency validation of clinical staff in compliance with Medicare and Medicaid Conditions of Participation
  • Oversee individualized patient care plans, care conferences, and record audits for regulatory compliance
  • Coordinate interdisciplinary team services, physician collaboration, and internal referrals
  • Monitor quality and compliance through QAPI, infection prevention, and emergency preparedness activities
  • Manage staff scheduling, clinical resources, and equipment to support effective operations
  • Ensure accurate, timely clinical documentation in the EMR
  • Maintain availability for consultation, on-call coverage, and direct patient supervision as needed
  • Investigate and report incidents related to patient care, safety, or employee well-being

Other Duties:

  • Participate in meetings, trainings, and performance reviews as required
  • Adhere to infection control, safety, and risk management procedures appropriate to the office environment
  • Document relevant communication and administrative activity accurately and in a timely manner
  • Maintain compliance with HIPAA and all company privacy and confidentiality standards
  • Provide courteous, timely, and professional customer service to internal and external stakeholders
  • Work independently with minimal daily oversight and demonstrate initiative in completing responsibilities
  • Exhibit adaptability to changing responsibilities and flexibility in completing assignments
  • Demonstrate consistent and effective use of organizational technology systems and tools required for the role, including communication, documentation, productivity, and data management platforms
  • Adhere to all organizational policies, including job descriptions, mission, and the Employee Handbook
  • Communicate and interact professionally and respectfully with others to support team goals
  • Perform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the company

Requirements

 

What We're Looking For

  • Graduate of an accredited nursing or therapy program; current, unrestricted New Mexico license required
  • Minimum 3–5 years of clinical experience, including at least 1 year in home health or a related setting
  • Demonstrated ability to supervise clinical staff, coordinate care, and maintain regulatory compliance
  • Strong knowledge of Medicare and Medicaid Conditions of Participation and state licensure standards
  • Proven leadership, communication, and organizational skills
  • EMR proficiency and comfort managing multiple clinical priorities
  • Valid driver's license and reliable transportation
