Under the general direction of the Director of Direct Care Services, the Youth Program Case Manager is responsible for providing comprehensive assessment, planning, implementation, and overall evaluation of individual client needs to youth ages 18-24 experiencing homelessness, as well as working with veterans.

Qualifications Required:
- Ability to understand and practice the mission of Camillus House.
- Education: Bachelor’s Degree preferred in Human Services or related field; any appropriate combination of education, experience and/or certification will be considered from an accredited College or University or in lieu of degree 3+ years of relevant experience may be considered.
- Experience: Minimum one year experience in same or similar job role is preferred.
- Skills: Proficiency in computer (Word, Excel, other data collecting programs).
Qualifications Preferred:
- Proficiency in oral/written English; Spanish preferred; additional languages a plus (i.e. Creole).
- Culturally sensitive to the social, psychological, and healthcare needs of the vulnerable populations served.
- Experience working in healthcare, social services, nonprofit, or customer service setting.
- Coordinates intake of eligible clients.
- Assesses clients’ challenges as well as strengths.
- Works with participants to identify goals and develop case plans and facilitate participants’ work towards achieving the goals identified in the case plans.
- Conducts independent living skills assessments with youth to determine their needs and make appropriate referrals.
- Provides clients with referrals and monitors progress as needed, in particular to mental health and substance abuse services, and towards opportunities for educational and career development and personal enrichment.
- Supports clients in identifying permanent housing options.
- Prepares and submits reports in a timely manner.
- Coordinates the development of cooperative relationships with other services providers and any other community resources.
- Assesses housing service plans as needed, and if necessary, works with case managers to devise a plan that will meet the clients’ needs and the criteria of the program.

Compliance:
- Successful completion of toxicology test, criminal background check and of a Level II screening.
- Valid FL Driver License with clean driving record is required for this position.
- Demonstrated knowledge of and compliance with organizational policies and procedures, accreditation standards, licensing requirements, and applicable federal, state, and local laws and regulations related to the position.
- Maintains high ethical and professional standards and performs duties in accordance with organizational mission, vision, values and code of conduct.
- Completes all required training, certifications, and compliance requirements within established timeframes.
Work Schedule: Onsite
- 8:00 AM-4:30 PM or 8:30 AM-5:00 PM
- Ability to work a flexible work schedule. Shifts may vary (evenings and weekends as needed).
- Volunteer at annual fundraising events.
