Social Worker for Skilled Nursing

Full-time exempt position, excellent benefit package after 60 days including medical, dental and vision.  Paid vacation and sick time, holiday pay, daily meal allowance, 403b match up to 6%, and other benefits.

Key Responsibilities:

- Conduct psychosocial assessments of residents to identify their needs, preferences, and potential barriers to care

- Develop and implement individualized care plans in collaboration with residents, families, and healthcare teams

- Provide emotional support to residents and their families to help them cope with health challenges and transitions

- Coordinate with community resources, agencies, and support services to meet residents’ social and emotional needs

- Advocate for residents’ rights and preferences within the healthcare setting

- Document all interactions, assessments, and care plans accurately and in compliance with regulatory standards

- Participate in interdisciplinary team meetings to ensure comprehensive resident care

- Educate residents and families about available services, benefits, and community resources

Skills and Qualifications:

- Bachelor’s degree in Social Work (BSW) or related field; Master’s degree preferred but not required

- Valid state licensure or certification as a Licensed Social Worker (LSW) or equivalent

- Proven experience working in a healthcare or skilled nursing environment experience preferred

- Strong interpersonal and communication skills, with the ability to build rapport with diverse populations

- Knowledge of healthcare regulations, Medicaid/Medicare policies, and community resources

- Ability to handle sensitive situations with empathy, professionalism, and discretion

- Excellent organizational and documentation skills

- Ability to work collaboratively within a multidisciplinary team
