Reports to: The DoDS
Supervision of: All staff
Hours: 40 hours per week
Values: Site Supervisors are responsible for exhibiting and displaying the agency’s mission and basic philosophy of consumer validation and empowerment through respectful interaction. They must ensure that program staff adheres to agency policies and procedures and DDS regulations
Responsibilities:
All responsibilities are under the direction, supervision and in conjunction with the Program Director
- Responsible for the day to day scheduling and functioning of the programs. Ensure a focus on learning new skills through innovative and creative teaching lesson plans for groups that include age appropriate skills for adults. Oversee all transportation scheduling.
- Ensure quality care of all individuals. Model and lead using a PBS approach. Ability to de-escalate crisis situations
- Ongoing training and supervision of staff.
- Hiring, training, supervision of staff. Ongoing leadership using a team approach of empowerment and learning through PBS, via staff meetings at least twice monthly and daily accountability.
- Attends and provides input into Directors’ Meetings or ensures program representation. Disperses information from meetings to all staff in the program
- Participate in PBS committees and development, implementation, and oversight of PBS techniques.
- In Cooperation with DoDS, evaluation of all staff, including relief
- Provides adequate program coverage and back up to the staff for emergencies and/or arranges for another to be available in their absence
- Complete facilitation of the staff meetings twice monthly
- Human Rights Officer for the program. Must attend training to become certified and represent the program at quarterly HRC meetings. Must also be available to address potential human rights issues with consumers
- Maintain current mandatory training certifications including CPR/First Aid, MAP, PBS, HRO, Formal Fire Safety, MAP for Supervisors,
- Assisting in completion of individual’s ISP including required assessments
- Attends ISP meetings with the DoDS to determine goals, develop PSS and data forms and forward to DDS within 10 days of the ISP meeting
- In Cooperation with DoDS, responsible to oversee all documentation especially the Confidential Files.
- Responsible for communication log, staff timecards, request for time off, client data sheets, daily and monthly progress notes, medication sheets
- Assign and supervise case management for monthly progress notes, program supply shopping
- Responsible to ensure that all medications and refills are in place and ready for administration
- Responsible for turning in all receipts and required paperwork to the office in a timely manner
- Forward all required materials to DDS personnel, day programs, families & other government agencies
- Provide a clean, safe and attractive program for the individuals. Maintain relationship with landlord
- Represent their programs and/or individuals when dealing with DDS Service Coordinators and Office of Quality Enhancement as well as other state agencies
- Establish and maintain ongoing family contact
- Organize and implement group and individual activities which serve to expand the lives of individuals and foster greater degrees of independence
- Complete, document fully quarterly Fire drills, and additional drills for new referrals.
- To perform other related work duties as needed or as assigned
Requirements
QUALIFICATIONS:
- BA or BS in Human Service discipline preferred OR 6 years’ experience in related field.
- At least 3 years of increasingly more responsible positions with a human service agency
- Supervisory experience
- Graduate of DDS Leadership series or willingness to complete preferred,
- Excellent written, verbal, and interpersonal skills.
- Must possess a working knowledge of ISP process and Survey & Certification standards.
- Proficiency utilizing Microsoft Office, video conferencing and the internet
- Manages multiple priorities and resolves issues in a calm and pro-active manner
- Work cooperatively and effectively with others to set goals, resolve problems, and make decisions that enhance organizational effectiveness
- Valid Driver’s License
- Successful completion of all state and agency mandated trainings including Medication Administration, CPR, First Aid
- Must be willing to authorize a Criminal History Background Check (CORI), and Fingerprinting check through National Criminal Background Check (NCBC) and release of information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies

Hours may alter to meet the needs of the program. Ten calendar days notice given
