SCOPE OF ROLE

Reporting to the Supervisor, Behavioral Health, the Counselor, Outpatient & Crisis Services provides outpatient counseling, crisis intervention, crisis assessment, and care coordination services to adults and children experiencing behavioral health and emotional difficulties. The Counselor, Outpatient & Crisis Services utilizes a team-based, trauma-informed treatment planning approach to provide exceptional therapeutic intervention, crisis stabilization, assessment, referral, and continuity of care services. This role combines ongoing counseling responsibilities with urgent behavioral health response and crisis management functions. Services are provided at Signature Health locations, client homes, healthcare settings, schools, and other community-based environments as clinically appropriate; provided individually, in group settings, or with families.

 

HOW YOU’LL SUCCEED ____________________________________________________________________

  • Conduct individual, group, and family therapy for adults and children. 
  • Develop and implement person-centered treatment plans utilizing evidence-based practices. 
  • Assist clients in preparing and presenting treatment program work material. 
  • Prepare all documentation required for discharge planning and aftercare referral. 
  • Deliver services using telehealth modalities when appropriate and approved. 
  • Meet established productivity, documentation, and service delivery expectations. 
  • Provide crisis assessments and crisis intervention services in homes, community settings, healthcare settings, correctional facilities, and Signature Health locations. 
  • Conduct crisis prescreening and diagnostic assessments to determine the appropriate level of care and disposition for individuals experiencing behavioral health emergencies. 
  • Assess risk related to suicide, homicide, self-harm, substance use, trauma, and psychiatric instability and develop appropriate safety plans. 
  • Coordinate crisis stabilization services and referrals to inpatient, residential, outpatient, and community-based resources as indicated. 
  • Utilize sound clinical judgment during emergency situations to ensure the safety and welfare of clients and others. 
  • Respond effectively to urgent behavioral health situations while maintaining professionalism and therapeutic engagement. 
  • Obtain and maintain appropriate Releases of Information to facilitate coordination of care. 
  • During crisis interventions, function as a liaison between Signature Health and hospitals, criminal justice systems, schools, social service agencies, community providers, and family support. 
  • Coordinate follow-up care and facilitate timely linkage to treatment and community resources. 
  • Collaborate with multidisciplinary treatment teams to optimize client outcomes. 
  • Participate in community outreach and engagement activities supporting Signature Health initiatives. 
  • Maintain client documentation in accordance with Medicaid, Medicare, DBH, CARF, Joint Commission, and professional practice standards. 
  • Actively engage in clinical supervision and professional development. 
  • Consult with leadership regarding ethical, legal, and clinical concerns. 
  • Demonstrate a commitment to quality care, service excellence, and continuous improvement.

  • Perform other duties as assigned. 


Requirements

KNOWLEDGE & EXPERIENCE ______________________________________________________________

  • Bachelor’s Degree required. Master's Degree in Counseling, Social Work, Marriage and Family Therapy, or related behavioral health field preferred. 
  • Valid unencumbered LSW, LISW, LPC, LPCC, LIMFT or MFT License in the state of Ohio required.
  • American Heart Association (AHA) Basic Life Support (BLS) certification required. 
  • Demonstrated competency in the treatment of mental health and substance use disorders. 
  • Experience conducting crisis intervention, risk assessments, safety planning, and behavioral health evaluations preferred. 
  • Working knowledge of community behavioral health systems and crisis continuum services. 
  • Experience collaborating with interdisciplinary teams to improve client outcomes. 
  • Familiarity with state and federal healthcare regulations, documentation standards, and behavioral health best practices. 
  • Knowledge of community resources available to individuals and families experiencing behavioral health crises


WORKING CONDITIONS ___________________________________________________________________

  • Work may be performed in a typical interior/office/clinical work environment, in client residences, and in the community.
  • While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.
  • Possible exposure to blood borne pathogens while performing job duties.
  • Requires periods of sitting, standing, telephone, and computer work.  
  • Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.
  • Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.
  • Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.
  • Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.
  • Possible exposure to blood borne pathogens while performing job duties.
  • Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.
  • Sufficient dexterity to operate a PC and other office equipment.
