Talbot Hospice is seeking a compassionate and experienced Registered Nurse, Clinical Liaison to serve as the connection between hospitals, healthcare providers, patients, families, and hospice services. This role focuses on assessing patients for hospice eligibility, coordinating referrals and admissions, educating healthcare partners, and ensuring a smooth transition into hospice care.


Key Responsibilities

  • Assess patients for hospice eligibility and coordinate admissions.
  • Partner with hospital staff, case managers, physicians, and referral sources.
  • Educate patients, families, and healthcare professionals about hospice services.
  • Coordinate timely admissions, transportation, medications, and equipment.
  • Complete clinical assessments and documentation.
  • Provide compassionate patient care and family support.
  • Collaborate with the interdisciplinary team to ensure continuity of care.

Requirements

POSITION QUALIFICATIONS

  1. Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care.
  2. Current licensure in State of Maryland and CPR certification.
  3. Bachelor’s degree, with one (1) year of hospice care experience preferred.
  4. Management experience not required. Responsible for supervising hospice aides.
  5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
  6. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
  7. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

 

Salary Description
$38 to $45 per hour
