SUMMARY:
The Residential Case Manager works as part of the interdisciplinary treatment team to coordinate discharge planning and support clients transitioning from residential treatment to community-based services. The Case Manager identifies barriers to successful discharge, coordinates referrals, connects clients with community resources, and assists with developing individualized aftercare plans.
The Case Manager works under the supervision of the Program Director and Clinical Supervisor/Therapist to promote continuity of care, recovery, and successful community reintegration.
DUTIES AND RESPONSIBILITIES:
The Case Manager’s responsibilities include, but are not limited to:
- Initiate discharge planning at admission and update plans throughout treatment.
- Assess client needs, strengths, barriers, and goals related to successful community reintegration.
- Develop individualized discharge plans in collaboration with clients and the treatment team.
- Identify and address barriers related to housing, transportation, insurance, identification, and access to care.
- Coordinate referrals and follow-up appointments for outpatient treatment, behavioral health, medical care, and recovery support services.
- Collaborate with community providers, referral agencies, and authorized family or support persons to promote continuity of care.
- Facilitate warm handoffs and coordinate transitions to appropriate services following residential discharge.
- Connect clients with housing, employment, education, benefits, transportation, childcare, and other community resources.
- Assist clients with applications, documentation, appointments, and resource navigation.
- Support clients in identifying recovery supports and developing practical skills for independent living.
- Maintain current relationships with community organizations and referral partners.
- Participate in treatment team meetings and discharge planning discussions.
- Provide regular updates to the Program Director and Clinical Supervisor/Therapist regarding client progress, discharge barriers, and resource needs.
- Consult with clinical leadership regarding concerns affecting discharge planning and follow clinical recommendations.
- Escalate urgent needs, safety concerns, and significant changes in client circumstances to appropriate supervisors.
- Maintain timely and accurate case management notes, referrals, assessments, and discharge planning records.
- Obtain appropriate releases of information before coordinating with outside providers or supports.
- Maintain confidentiality and comply with HIPAA, 42 CFR Part 2 when applicable, state regulations, and agency policies.
- Complete post-discharge follow-up as required by agency policy.

- Proficient use of standard office equipment and systems: phone, copier, computer including Microsoft Office Word and Outlook tools.?
- Ability to coordinate volunteer and vocational activities for participants?

- Ability to manage a variety of tasks and organize workload?
- Attention to detail?
- Maintain effective professional relationships with all levels within the organization?
- Familiarity with community resources including mental health, social, financial, educational, housing, employment, childcare, and other support that may be required.?
- Ability to build rapport with clients?
- Unrestricted NM Driver’s License
Education and Experience:
- High School/GED required
- Experience working with individuals with serious mental illness (SMI), substance use disorders (SUD), co-occurring disorders, children and families, or vulnerable populations preferred. (1 year)
- Case Management certification (Required) - ACM (Accredited Case Manager), CCM (Certified Case Manager, RN-BC), or CPSW (Certified Peer Support Worker)?
- Current CPR/First Aid Certification (or willingness to become certified)?
