RN FIELD NURSE (Float)

Shift: Mon-Thurs 7a-5p

Overview:  The Float RN provides flexible clinical coverage for hospice patients covering for planned and unplanned Nursing absences. This role works independently in the field, delivering direct palliative nursing care to terminally ill patients and their families. The Float RN follows an established, medically approved plan of care and ensures safe, compassionate, and compliant hospice services across the caseloads they support. 

Reports to: Director of Nursing 

Responsibilities/Essential Functions:

• Prepare for shift by reading Nurse shift reports, triage reports, and Microsoft Teams channels and chats to assess  urgent needs. 
• At start  of shift, be ready and available to perform visits and assignments as      requested by calls coming into the office, RNCM, triage, Director of      Nursing, or Administrator/Administrator On Call
• Assess and monitor pain, administer      medication, provide wound care and manage symptoms
• Observe and assess patients through data      sources such as weight, pedal edema, patient history and input, and  respiration. Report observations and changes to RNCM or Physician.
• Educate patient and family about  realistic expectations of disease and dying process
• In-person presence and support for  patients, families, and facility partners
• Complete medical charting correctly and  appropriately for the needs of Medicare, State Health Department, ACHC,  and per established standards.
• Review and document patient needs in care   plans, document and address problems, update medications
• Complete initial patient assessments 
• Ensure that patients are admitted timely  and properly with correctly completed documentation
• Communicate with care team and other caregivers to coordinate services of hospice care
• Present hospice philosophy and services; explain patient rights and responsibilities.
• Participate in meetings, in-services, and other required training (24-30 hours per year)
• Other interventions based on patient/family needs.
• Travel required
• Other duties as assigned

Qualifications/Educational Requirements:

• Must possess current, unencumbered, active license to practice as a Registered Nurse (RN) in the State of Colorado.
• Must  have valid car insurance and driver’s license.
• Knowledge of and experience in pain and symptom management.
• Well-developed  physical assessment skills.
• Must be able to read, write, speak, and understand the English language.
• Must possess patience, tact, and a positive attitude.
• Hospice experience preferred.

Work Environment:

General offices, long-term care facilities, assisted living facilities, and patient home settings will be the primary locations of the Nurse’s activities. Exposure to infectious diseases is possible and universal precautions should be taken at all times. Travel is a requirement of this position (with mileage reimbursement).
 

Scheduling
The expected availability for this position is Monday-Thursday from 7a-5p (4 x10 hour shifts). The Float RN is expected to work a minimum of 2 holiday shifts per year and will receive $300 for those shifts (in addition to regular pay) and any other on-call/after-hours shifts picked up.  

Compensation & Benefits

Includes a salary of $88,500 - $97,800 annually, mileage reimbursement at IRS rate, and company-provided cell phone. Dignity Hospice pays 90% of the employee premium towards the base health insurance plan (United Healthcare), dental, and vision. 48 hours of SICK time per year, and 80 hours of PTO in the first 12 months, increasing over time with tenure at 2 years and 5 years. 401(k) with company match is available after 12 months.
