The Bi-Lingual Eviction Prevention Case Manager provides direct services to tenants in arrears of their rent and at risk of homelessness including, but not limited to: participation in screening of potential clients through our weekly hotline, maintaining a schedule of weekly meetings with clients, as well as case planning to assist tenants in resolving their arrears and the factors that may have led each tenant to fall behind. The case manager works as a member of the Crisis Services team and reports directly to the Director of the Eviction Prevention Program. The case manager performs job duties consistent with the mission and goals of the Coalition for the Homeless and the terms of relevant funding agreements.
The Bi-Lingual Case Manager shall:
1. Assess prospective client’s cases via our weekly hotline to ascertain that clients are indeed at immediate risk of eviction and that there exists the future ability to pay rent should they receive financial assistance;
2. Help clients to understand and navigate housing court and how eviction prevention services work if necessary;
3. Assist clients to gather all the necessary documentation to process their grant;
4. Assess cases for the need for additional assistance and make referrals as needed for services, including but not limited to, benefits and entitlements, legal assistance, mental health and substance use services;
5. Support and provide information to clients in advocating for their rights and benefits;
6. Collaborate and advocate with other member agencies of the Emergency Rent Coalition, and Human Resources Administration whenever possible, to maximize the financial assistance provided to clients;
7. Process cases for eviction prevention grants as efficiently as possible, meeting case processing goals and funding requirements;
8. Develop and maintain a working knowledge of relevant social services systems/agencies, including housing programs and procedures, the housing court process, as well as benefits and entitlement programs;
9. Maintain accurate and proper record documentation, such as case management notes, and ensure that the program files and the client database are up to date;
10. Inform the Eviction Prevention Program Director and Senior Director of Crisis Services of client emergencies and work with to address them;
11. Work in conjunction with other Crisis Services programs to exchange information relevant to Crisis Services, and collaborate with other Coalition departments and legal service providers as necessary;
12. Obtain regular (no less than weekly) supervision with the Eviction Prevention Program Director to review client issues/concerns and service needs;
13. Assist with mailings, phone calls and other duties as they relate to client follow-up efforts;
14. Participate in staff meetings, such as team meetings, in-service training, staff development and orientation activities;
15. Provide Eviction Prevention Program Director with input and client information for any necessary reporting relevant to funding requirements;
16. Assist in monitoring and maintaining a professional, respectful, and courteous work environment;
17. Perform all other duties as may be assigned by the Eviction Prevention Program Director or Senior Director of Crisis Services.
AA/AS in human services (or a GED/High School Diploma and at least 2 years relevant work experience in a social service field). Prior case/care management experience required and experience working with individuals and/or families who have experienced homelessness, people living with mental illness, and/or persons living with disabilities preferred. Prior experience with client advocacy strongly desirable. Spanish fluency required.
