Program/Department Summary
The Community Health and Wellness sector provides services that support the physical, behavioral, and emotional health and wellbeing of individuals and communities. Our staff are certified Traditional Health Workers (such as Community Health Workers (CHW), Peer Support Specialists (PSS), and Doulas) who offer direct services such as healthcare and systems navigation, health insurance enrollment, physical and emotional health support, adjustment support, mental health resource navigation, and client-directed case management. We also engage in community outreach and physical and emotional health education via community workshops, psycho-education support groups, and by hosting monthly health clinics that provide dental services, health services, and community resource information.
Position Summary
The Health Related Social Needs (HRSN) Program Coordinator is responsible for coordinating the daily operations of IRCO’s Portland Metro HRSN Housing and Outreach and Engagement programs, including directly supervising a team of case managers who work with clients to provide rent and utility assistance, tenancy support, and resource navigation.
The HRSN Program Coordinator provides day-to-day supervision and mentoring of staff, administrative duties involved in rental and utility payments and reimbursement, and program oversight ensuring trauma-informed services reach community members. This position ensures staff have ongoing training and competence to provide referrals, direct services, and support to assist participants and provide accurate documentation of services as required.
This position works closely with the Community Health & Wellness Sector Manager and Coordinators team, program funders and community partners. The HRSN Program Coordinator successfully coordinates, implements, reports, and evaluates the program in their portfolio, as well as other projects as needed and assigned.
Essential Functions
- Possess and apply an impeccable degree of confidentiality and utilize knowledge of laws about confidentiality, disclosures, and releases of information to ensure program and staff compliance.
- With a fair degree of autonomy, coordinate implementation of the HRSN program, develop staff workplans, monitor caseloads and opening/closing of new cases, monitor expenses, monitor ongoing program eligibility (Oregon Health Plan enrollment), review and submit invoices, track deliverables and prepare reports and communicate with funders as needed.
- Supervise a team of case managers; hold regular case conferences and utilize problem-solving skills and program expertise to support and grow staff; provide staff with backup during absences or difficult client situations.
- Implement a participant-centered, trauma-informed model to lead staff in assisting clients to attain their long-term housing goals.
- Ensure confidential participant records are recorded and maintained.
- Ensure proper service delivery documentation and case notes are completed to award or invoicing standards.
- Approve and process requests for rental and utility assistance ensuring all funder and IRCO requirements for payment requests are followed; all program required financial tracking is completed and to work closely with IRCO’s Finance team on all financial matters.
- Onboard staff; lead performance evaluations in alignment with agency practices; ensure consistent interpretation of IRCO policies and internal guidelines; make recommendations to Community Health & Wellness Sector Manager on hiring and disciplinary actions when needed.
- Foster a supportive, positive, and accountable work environment.
- Regularly review participant files and service records, ensure data entry, case notes, updates, reports, timesheets and all other administrative tasks are completed accurately within given timelines.
- Periodically update protocols and procedures, with guidance from Community Health & Wellness Sector Manager and other teams.
- Research and share relevant trainings and professional development opportunities for HRSN staff and maintain up-to-date community resources and referral information to support client services.
- Build relationships with other IRCO programs and external service providers to ensure participants have access to appropriate, high-quality referrals.
- Participate in internal and external meetings as requested, with the goal of creating stronger connections for client services and advocating for client needs.
- Coordinate or conduct ongoing training of staff relevant to the HRSN program and their roles.
- As requested, prepare and conduct culturally and linguistically specific presentations on IRCO’s HRSN programs.
- Perform other duties and lead other projects as assigned.
Education & Experience
MINIMUM JOB SPECIFIC QUALIFICATIONS:
- Bachelor’s degree preferably in public health, social work, counseling, and/or human services related field.
- Minimum 2 years of experience working in social services, preferably in housing programs.
PREFERRED QUALIFICATIONS:
- Experience and knowledge of Oregon Medicaid programs or billing and the Health Related Social Need Waiver preferred.
- Experience and knowledge of housing programs and laws, tenant rights and housing systems preferred.
- Understanding of the social determinants and impacts of health and its intersections with violence, poverty, and homelessness.
- Understanding of Trauma-Informed Care and experience with reflective supervision.
- Must have knowledge and understanding of immigrant and refugee populations and challenges/dynamics; cultural awareness and sensitivity.
- Ability to cultivate strong working relationships with stakeholders, colleagues, and community members.
- An effective and compassionate communicator, both verbally and in writing, with excellent active listening skills to identify the needs and goals of our participants.
- Highly organized, self-directed, and with the capacity to multitask and pivot effectively; able to meet multiple and often competing deadlines.
- Strong analytical, problem solving, and decision-making skills.
- Able to respond to stressful situations balancing individual circumstances with organizational policies and program goals.
- Ability to write program reports that are accurate, clear, concise, and effectively convey program accomplishments.
- Strong skills in tracking outcomes/outputs; generating, reviewing, and/or analyzing reports.
- Strong skills and experience in tracking, reviewing and analyzing program expenses, processing program related payments and expense requests, and creating and managing complex budgets.
