Leaving jail can mean navigating housing, employment, health care, treatment, benefits, transportation, and other basic needs—all at once.
As a Reentry Case Manager with Bridgeways’ Jail Transition Program, you’ll work with adults preparing to return to the community and after release, helping them identify needs, connect with resources, and work toward greater stability.
This is a community-based role for someone who enjoys working independently, building relationships, solving problems, and turning “this person needs help” into “here’s our next step.”
What you’ll do:
You’ll independently manage a caseload, typically 20-30 clients, with support from your supervisor and team.
- Meet with clients in local jails and throughout Snohomish County to assess needs and plan for successful reentry.
- Connect clients with housing, behavioral health and substance use treatment, health care, employment, benefits, transportation, and other community resources.
- Develop transition plans, monitor progress, troubleshoot barriers, and follow up to keep things moving.
- Build relationships and coordinate with jail staff, community providers, government agencies, and other partners.
- Advocate for clients and provide support when challenges arise.
- Maintain timely case notes and documentation in the electronic medical record (EMR).
- Manage your own schedule, appointments, priorities, and follow-ups.
Schedule & location:
This is a full-time position with a typical Monday–Friday daytime schedule, generally between 8:00 a.m. and 5:00 p.m., with some flexibility based on personal, client, and program needs.
Your home office will be in Everett, WA, with regular travel throughout Snohomish County to meet clients and community partners, including visits to local jails and community settings.
Company vehicles are available for work-related travel for employees meeting Bridgeways’ driving requirements.
Pay & benefits: $27.65 – $29.70 per hour

What we’re looking for:
We’re looking for someone with case management skills who is comfortable taking initiative and working independently. You may be a good fit if you:
- Have experience providing case management or direct support to people navigating complex needs.
- Are comfortable meeting new people and building rapport with individuals from a wide range of backgrounds and circumstances.
- Are comfortable working within jail and criminal justice settings.
- Can manage multiple clients, appointments, deadlines, and follow-ups without needing someone else to continually direct your day.
- Are proactive about connecting people with resources—you’re willing to make the connection, follow up, troubleshoot barriers, and help determine the next step.
- Have basic computer skills and are comfortable communicating by email, phone, and in person.
- Are able to travel to required work locations throughout Snohomish County.
- Are able to meet applicable background and jail-access requirements.
- Are authorized to work in the United States.
Experience with reentry, behavioral health, substance use recovery, housing or homelessness services, outreach, benefits navigation, criminal justice, or community-based social services is especially helpful.
We consider the combination of education, professional experience, volunteer experience, and lived experience that has prepared you to provide effective case management and work with people experiencing complex barriers.
Relevant education may include social work, psychology, criminal justice, human services, or a related field.
Certifications such as Certified Case Manager, Mental Health First Aid, or Peer Counselor are welcomed but not required.
