Minnesota Assistance Council for Veterans (MACV) is a statewide nonprofit organization with a mission to end Veteran homelessness in Minnesota. MACV provides comprehensive programs in the areas of housing, employment, and legal services to support Veterans and their families throughout Minnesota who are homeless or at risk of homelessness. MACV operates a budget over $14 million annually and has a team of approximately 100 employees throughout Minnesota. The organization has office locations throughout the state, with administrative functions centralized within a headquarters office based in St. Paul.

Position Overview:

 

This position serves as a primary point of contact for Veterans seeking assistance from MACV and for community partners making referrals. The Intake Coordinator is responsible for assessing eligibility, gathering necessary information, coordinating referrals, and ensuring Veterans are connected to appropriate services in a timely manner.


The Intake Coordinator must be able to communicate professionally with a wide range of individuals, manage multiple priorities, and remain organized and composed in challenging or rapidly changing situations. The ideal candidate is professional, poised, resourceful, and able to use sound judgment while balancing the needs of Veterans, community partners, and internal staff.


Primary Duties and Responsibilities

  • Conduct initial screening and assessment of Veteran eligibility for MACV services through phone, email, walk-in contacts, website requests, and referrals from community partners. 
  • Review and assess information to determine appropriate next steps and connect Veterans with MACV or other community resources. 
  • Process, document, and forward eligible referrals for assignment to appropriate case management staff. 
  • Maintain accurate and timely documentation of Veteran contacts, referrals, dispositions, and services in shared databases and client tracking systems. 
  • Communicate eligibility determinations and referral outcomes to Veterans and community partners as appropriate. 
  • Obtain additional documentation or information from Veterans and referral sources as needed to facilitate timely assistance. 
  • Respond professionally and effectively to Veterans, community providers, and other stakeholders, including during situations involving urgency, uncertainty, or competing needs. 
  • Identify barriers to assistance and use sound judgment and problem-solving skills to determine appropriate next steps. 
  • Manage multiple referrals, communications, and administrative responsibilities simultaneously while maintaining accuracy and attention to detail. 
  • Maintain client confidentiality in accordance with MACV policies and applicable regulations. 
  • Provide administrative support to the Northern Region as needed, which may include financial, operational, reporting, and other organizational processes.

  •  Experience working with databases, client tracking systems, or other information management systems. 
  •  Learn new systems and processes quickly and use technology confidently to manage information and workflows. 
  •  Ability to work effectively both independently and as part of a team. 
Requirements

Preferred Experience:

 

  • Experience with HMIS or other homeless services/client information systems. 
  • Knowledge of Veteran services, Veteran issues, or community-based resources. 
  • Experience working in a human services, nonprofit, healthcare, social services, or similar environment. 
  • Experience coordinating referrals or navigating individuals through multiple service systems. 
  • Familiarity with local community resources and service providers. 
  • Experience working with individuals experiencing housing instability or other significant barriers to stability.
  • Valid Driver’s License
