As a Supported Living Manager-2 at Pathfinder, you’ll play an important leadership role in helping people with intellectual and developmental disabilities live, learn, work, and participate as active members of their communities. You’ll support the daily operations of two supported living locations or a caseload of approximately 5–12 people, while leading and developing the Direct Support Professionals (DSPs) who provide their day-to-day support.
This is a hands-on leadership role where your organization, communication, advocacy, and commitment to person-centered support will directly impact the quality of life of the people we serve.
As a Supported Living Manager-2 at Pathfinder, You Will:
Lead with Person-Centered Support
- Build positive, respectful relationships with people supported, DSPs, families, guardians, and members of the interdisciplinary team.
- Keep person-centered thinking at the heart of all services and decisions.
- Support people in making informed choices and achieving the goals outlined in their Individual Support Plans (ISPs).
- Model and encourage person-centered thinking tools and practices with employees and people supported.
- Advocate for the needs, preferences, rights, and quality of life of the people you support.
- Help individuals build natural support systems and meaningful connections within their communities.
Lead & Develop Your Team
- Provide leadership, supervision, training, mentoring, and direction to DSPs.
- Ensure DSPs consistently provide quality, person-centered support.
- Train staff on ISP objectives, behavior plans, prevention of abuse and neglect, personal assistance, residential habilitation, and person-centered planning.
- Hold staff meetings and provide ongoing coaching and guidance.
- Review and approve DSP timesheets and complete personnel responsibilities as needed.
- Work closely with the Call Center to maintain appropriate staffing coverage.
Coordinate Daily Supports & Services
- Coordinate staffing, medical appointments, transportation, work, shopping, recreational activities, and other daily needs.
- Ensure ISP goals, objectives, behavior plans, and safety measures are being implemented consistently.
- Work with DSPs to turn everyday activities into opportunities for learning, independence, and skill development.
- Ensure appointments are scheduled and communicated appropriately to day programs and other members of the support team.
- Participate in Individual Support Plan, Person-Centered Planning, six-month, and annual meetings.
- Serve as a primary point of contact for members of the interdisciplinary team, including guardians, families, case managers, job coaches, and other community partners.
Support Health & Medication Needs
- Ensure medications are administered, documented, and maintained according to Pathfinder policies and procedures.
- Make sure prescriptions and necessary medication supplies are available.
- Coordinate with nursing staff regarding medical needs and communication.
- Ensure medical appointments are scheduled and followed through appropriately.
- Bring significant medical or safety concerns to the appropriate Pathfinder leadership or nursing staff.
Documentation & Compliance
- Ensure required documentation is completed accurately and on time.
- Oversee documentation related to Medicaid, DDRS, BQIS, ISPs, medication records, activities, behavior, nutrition, and personal spending.
- Maintain accurate Medicaid Waiver Supported Living files.
- Complete quarterly summaries and required reporting.
- Support internal and external audits by providing documentation and responding to requests for information.
- Participate in developing and implementing corrective action plans when needed.
- Ensure services and supports meet Pathfinder standards and applicable oversight requirements.
Home & Resource Management
- Monitor the overall condition and maintenance of supported living homes.
- Coordinate necessary repairs and communicate major maintenance or expense needs to leadership.
- Promote the efficient use of human, financial, and program resources.
- Work with leadership and case management to maximize available supports and services.
- Maintain appropriate financial documentation and support the financial needs of people served.
Flexible Staffing & On-Call Leadership
- Participate in an on-call rotation with other Supported Living Managers.
- Respond to after-hours calls and help address urgent staffing, home, or support needs.
- Help ensure open shifts at assigned homes are covered.
- Work with staff and the Call Center to find coverage when employees are absent or unavailable.
- Be willing to adjust your own schedule or provide coverage when needed to maintain appropriate staffing and quality services.
- At times, provide coverage or support for homes outside of your normally assigned responsibilities.
- Maintain flexibility as the needs of the people supported, employees, and homes change.

Preferred Minimum Qualifications:
- Minimum of a high school diploma/GED.
- Prefer 2 years experience in the field of Human Services.
- At least 18 years of age.
- Basic language, written, and computer skills necessary for communication and documentation.
- Demonstrated ability to support people with disabilities.
- Ability to work a flexible schedule if needed.
- Crisis prevention, intervention, and teaching skills.
- Valid driver’s license and current auto insurance required
- Ability to speak, hear, and read the English language.
