The Care Gaps Coordinator has duties and responsibilities related to preventive and follow-up patient care needs. The Care Gaps Coordinator conducts telephonic outreach to patients who are identified as needing preventive and/or follow-up care services in support of quality initiatives and regulatory/contractual requirements. The incumbent provides education to members regarding the need to eliminate their gaps in care. This individual will use telephone communication to assist in scheduling patients to visit the provider. The coordinator also assists in scheduling transportation services. The individual maintains confidentiality of business and protected health information.

Essential Functions:
- Conducts telephonic outreach to patients to support Quality Improvement, regulatory and contractual requirements.
- Consult with patients and family members to discuss their gaps in care.
- Educates the patient and their family about preventive health screenings while identifying barriers to care.
- Schedules appointments with medical providers to access needed preventive/follow-up care services and close gaps in care.
- Informs providers about the care gap(s) patients have and why they are scheduled for a specific visit.
- Documents all actions taken regarding contact related to the patient.
- Interacts with other departments and outside Insurance agencies to identify and resolve care gaps.
- Refers to Case Manager or Care Coordinator as appropriate.
- Communicates appropriately and clearly to team leader, coworkers and providers.
- Other duties as assigned.

Education: High School Diploma or GED Certificate required
Experience: National certification as a medical assistant or other healthcare professional preferred
- Experience in healthcare required
- Experience in a community health center highly desired
- Computer Skills: Proficiency with Microsoft Office applications (Word, Excel, Outlook, PowerPoint).
- Experience with an EMR desired
Other Requirements:
- Ability to communicate in English, both verbally and in writing.
- BLS Certification preferred
WORK ENVIRONMENT:
EX: Work is performed primarily in office, vehicles, and outdoor settings, in all weather conditions, including temperature extremes, during day and night shifts. Work is often performed in emergency and stressful situations. The noise level in the work environment is usually quiet in office settings and moderate in other situations.
