Position Title: Licensed Vocational Nurse (LVN)

Salary: $32 - $35 depending on experience


Summary

The Licensed Vocational Nurse (LVN) is responsible for coordinating the transition of patients to primary care after a hospital visit. The LVN is also responsible for completing care management co-visits with Virtual Clinic medical providers for specified medical conditions. On occasion, the LVN will also provide triage (sorting/prioritizing patients) over the telephone to assist in assessing patient care with medical provider.

JOB RESPONSIBILITIES:

  • Coordinates transition of care of patients from hospital setting into primary care setting and attempts to obtain all pertinent medical records prior to hospital follow-up visit.
  • Performs short evaluation of the patient situation to determine their medical needs and directs them to primary, support and/or referral services as needed.
  • Communicates with patients over the telephone to ensure follow-up care has been issued and/or completed.
  • Documents medical status information as needed.
  • Determines urgency of seeing a patient based on brief assessment and on familiarity with a patient’s condition and history. May confer with medical provider when needed.
  • Sets up appointments for patients to see a medical provider or arranges communication with a contact center representative to assist patient.
  • Serves as liaison between the clinic and Health Department regarding reportable illnesses.
  • Maintains patient trackers up to date.
  • Other duties as required by position.

EDUCATION & EXPERIENCE REQUIRED:

  • High school diploma or equivalent required.
  • Graduation from an approved vocational nursing program required.
  • Current, active state vocational nursing license required.
  • Bilingual: Spanish required.
  • At least one year of LVN experience in a high paced clinical setting preferred.
  • Qualified candidates must have strong interpersonal skills, must be able to maintain effective working relationships with co-workers, and effectively communicate and interact with diverse patient populations.

SKILLS & QUALIFICATIONS:

  • Knowledge of medical terminology, anatomy, physiology and pathophysiology.
  • Familiarity with health care systems, regulations, policies and functions, and documentation standards.
  • Knowledge of infectious disease management and control and safety standards.
  • Skill in initiating appropriate emergency procedures.
  • Ability to perform mathematical calculations for drug doses.
  • Ability to handle effectively multiple tasks simultaneously.
