We are seeking a mission driven and dedicated Community Health Worker to provide Enhanced Care Management services primarily to individuals experiencing homelessness and other individuals with needs. 

  • Over the phone, assess patients’ needs and connect them to appropriate resources. 
  • Improve patient satisfaction by calling patients to share with them their test results and provider's recommendation. At patient's request, schedule the patients for follow-up. Confirm patient's understanding and satisfaction. 
  • Willingness to assess patients for risk of HIV exposure and provide education on pathways for HIV testing and prevention.
  • Accurately and clearly document all patient interactions within patients’ medical records in a timely manner. 
  • Work positively as part of a collaborative team that includes an LVN, social worker, physicians, nurse practitioners, and community partner organizations. 

Essential Functions (What You Do).

The Community Health Worker is responsible for helping neighbors and health center patients navigate and access health center and community services, and aiding patients in adopting healthy behaviors. Community Health Workers promote health education resources, connect individuals to health insurance coverage and health care services, and assess the need for and connect individuals and families to community resources.

The position will also be responsible for:

  • Providing individuals with resources to obtain health insurance coverage and health care services.
  • Contact patients to administer the PREPARE assessment/Social Determinants of Health (SDOH) questionnaire, with the aim of identifying patients with the risk of potential HIV. 
  • Connect patients with educational resources that promote healthy living.
  • Assist patients with scheduling appointments, perform data entry such as updating medical history, demographics, and contact information.
  • Uphold all HIPAA (Health Insurance Portability and Accountability Act) and other regulatory compliance and safety requirements.

Requirements

Required Education:

  • High School Diploma or GED

Required License/ Certifications:

  • CPR/AED / BLS Certification
  • Valid and unrestricted California Driver’s License with clean driving record.

Required Experience:

  • 1 to 2 years of relevant work experience.

Preferred Experience:

  • Prior experience utilizing Epic electronic health record system Community Clinic experience.

Knowledge/Skills/Abilities:

Required:

  • Strong organization and follow-up skills.
  • Ability to multi-task and prioritize.
  • Knowledge of computer software programs and basic office equipment.
  • Demonstrates the ability to read, write and speak English and Spanish without an interpreter.
  • Demonstrates interpersonal/cultural sensitivity and work with a diverse and often underserved population.
  • Effective written and verbal communication across all levels of the organization and with vendors.
  • Highly organized with strong time management skills, being able to perform simultaneous tasks, prioritize them effectively and follow through quickly.
  • Proficiency with EMR systems. (EPIC experience preferred.)
  • Communication skills consistent with the facilitation of health and wellness topics to individuals, small and large groups of community residents.
Salary Description
$23.00 to 26.68 per hour
