Concept Rehab, a therapist-owned and operated rehabilitation company with more than 40 years of experience, is seeking a PRN Physical Therapist (PT) to join our team in Lima, Ohio.

As a Physical Therapist, you will evaluate and treat patients with mobility, strength, balance, functional, and positioning challenges while developing individualized treatment plans that maximize independence and quality of life.

Key responsibilities include:

• Evaluate patients for physical therapy services.
• Develop and implement individualized treatment plans.
• Provide skilled therapy interventions based on established plans of care.
• Supervise Physical Therapist Assistants in accordance with state practice regulations.
• Communicate effectively with physicians, nursing staff, families, and interdisciplinary team members.
• Participate in care planning and discharge planning activities.
• Document evaluations, progress notes, treatment notes, and discharge summaries accurately and timely.
• Educate patients, families, and facility staff on restorative and maintenance programs.
• Conduct in-service education and training for facility personnel.
• Maintain productivity and clinical quality standards.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in Ohio.
• Ability to meet all state and federal professional requirements.
• Experience in skilled nursing, post-acute care, rehabilitation, or geriatric settings preferred but not required.
• Current CPR certification preferred.
