The hospital registered nurse (RN) is responsible for the overall care of patients and families in the hospital setting. The hospital RN coordinates care delivery with interdisciplinary team members and directs the activities of other hospital staff.
Schedule: Full-time, 0.8 FTE, working 32 hours per week, typically from 8:30 a.m. to 5:00 p.m. The position includes working every fourth weekend and approximately 1–2 holidays per year.
What You'll Do:
- Build relationships with referral sources and hospital staff and provide hospice/palliative care education.
- Coordinate referrals, admissions, discharge planning, and transitions of care.
- Assess patients for hospice, palliative, and General Inpatient (GIP) appropriateness.
- Provide clinical assessment, symptom management, case management, and individualized care planning.
- Collaborate with physicians, NPs, and interdisciplinary team members to coordinate patient care.
- Assess patients in the Emergency Department and participate in hospital and interdisciplinary meetings.
- Administer and titrate medications and delegate/supervise appropriate nursing care.
- Maintain accurate documentation, clinical competency, confidentiality, and professional standards.
- Prioritize patient needs, maintain reliable attendance, and participate in 24/7 coverage as required.
- Follow all applicable laws, regulations, and organizational policies and complete required training.
- Perform other related duties as assigned.

1. Current unrestricted Registered Nurse licensure from the State of Wisconsin
2. One year RN experience preferred
