SCHEDULE: 40 hours, Monday - Friday, 7:00 a.m. – 3.30 p.m.; will require rotating on call coverage
Pays $25.12 - $29.00 per hour DOE, (Salary ranges provided are based on relevant experience and skill set)

Summary of the Position:
The Clinical/Lead Case Manager will be responsible for the overall case management of the residents of the Veterans Services program at 6 Hartford Street under the Behavioral Health Division of Pine Street Inn. The program serves veterans from diverse backgrounds, circumstances, and age groups and uses a Housing First/Stabilization model. The Clinical/Lead Case Manager will work in this transitional housing program for Veterans where the majority of individuals may be living with serious substance abuse, mental health issues, and/or physical disabilities. The Clinical/Lead Case Manager is responsible for program content including the implementation of effective best practices (Motivational Interviewing, Comprehensive Case Management, Harm Reduction, and Strengths-Based) and ensures the provision of high quality case management supports on a 24/7 basis. This will involve training and supporting the Case Manager at the second Veterans Services Program located at 8 Grove Street. The incumbent will be a great team player who is also self-motivated, with solid communications skills, capable of implementing new policies, procedures, and plans. This position requires a great deal of tact, diplomacy, flexibility and creative problem solving in order to maintain our positive working relationship with other departments and teams.

Education/Training:
Required:
- Associate’s degree in psychology or other related field such as counseling, psychosocial rehabilitation
- Preferred:
- A valid Massachusetts Driver’s License in good standing
- Bachelor’s degree in psychology or other related field such as counseling, psychosocial rehabilitation
- Advanced training in psychosocial rehabilitation and substance abuse
- Bi-lingual – Spanish/English
Knowledge/Experience:
Required:
- Minimum of three (3) years prior work case management experience working with homeless individuals suffering from complex disabilities in housing settings
- Minimum of at least (2) years prior experience working as Lead Case Manager and/or coaching and mentoring and/or providing supervision to staff in the human services and related fields
- Strong clinical background and familiarity with providing Motivational Interviewing and Harm Reduction Treatments to homeless and/or veteran populations
- Familiarity with the concepts of psychosocial rehabilitation and consumer empowerment
- Experience conducting psychosocial assessments
- Experience with case management and implementing treatment plans
- Experience coordinating care with HUD and Veterans Affairs
- Knowledge of treatments for chronic mental illness, including medications
- Previous experience working with dually diagnosed individuals
- Experience assisting people with ADL skills
- Experience working with other vendors and services within the public mental health system
- Previous experience moving homeless tenants into housing
Preferred:
- At least two (2) years of working in the capacity of a Case Manager
- Previous experience working with a Housing First/Stabilization model
- Familiarity with the Metropolitan Boston public and private mental health system
- Long term treatment experience with veterans, substance use, and/or mental health populations
