Position Summary
Wilmington Community Clinic (WCC) is seeking an organized, compassionate, and resourceful Integrated Care Coordinator to join our team. This position plays an important role in helping patients navigate healthcare and social services while ensuring their care is coordinated across WCC’s medical, dental, behavioral health, and outreach services. The Integrated Care Coordinator serves as a key point of contact for patients enrolled in care coordination, case management, Enhanced Care Management (ECM), and other care coordination programs. The ideal candidate is comfortable communicating with patients, providers, community partners, and health plans and is passionate about reducing barriers to care and improving the patient experience.
Compensation and Benefits
The compensation for this position is $22.00 - $25.00 hourly. WCC offers competitive salary and benefits including medical, dental and vision health insurance plans, 10 vacation days for first year employees, 12 paid holidays, sick leave, life insurance, a retirement plan, and an employee assistance program.
Education and Experience
- High School Diploma or GED; Bachelors Degree (Preferred)
- One year of general clerical and customer service experience
- Minimum of one to two years of experience in care coordination, case management, referral coordination, health navigation, or Enhanced Care Management (ECM) required; experience in a clinical, community health, or Medi-Cal managed care setting (Strongly Preferred)
Essential Position Responsibilities
- Serve as a central point of contact for patients participating in care coordination, case management, ECM, and other care coordination programs.
- Conduct patient outreach and follow-up after referrals to confirm understanding, encourage engagement, and identify barriers to receiving care.
- Track internal and external referrals from initiation through completion and ensure appropriate follow-up occurs.
- Document referrals, patient interactions, outcomes, care coordination activities, and program information accurately in electronic health records systems.
- Coordinate care across WCC’s medical, dental, behavioral health, and Outreach & Enrollment teams to support a whole-person approach to care.
- Communicate referral and care coordination updates to providers and care teams to support continuity of care and closed-loop referral management.
- Help patients overcome barriers to care by connecting them with emotional, financial, administrative, social, transportation, and community resources.
- Manage and track patient appointments, care plans, program enrollment, referrals, and interactions with WCC staff and external partners.
- Attend community events to connect patients to healthcare, social services, and care-management programs.
- Support patients in developing the skills and knowledge needed to independently navigate healthcare and social service systems.
- Coordinate transitions of care between WCC and outside clinical or social service organizations, including assisting with medical records and transportation needs when appropriate.
- Maintain relationships with community-based organizations and other resource providers to strengthen available support for patients.
- Participate in community events to promote WCC’s care coordination and case management services and develop community resource connections.
- Assist with the development, implementation, and enrollment activities associated with care coordination, case management, and ECM programs.
- Ensure program activities and documentation comply with Department of Health Care Services (DHCS), health plan, and HIPAA
- Participate in health plan audits, compliance reviews, meetings, and program specific trainings as needed.
- Maintain accurate patient demographic, appointment, referral, and program enrollment information.
- Promote awareness of available care coordination, case management, and ECM services among WCC care teams.
- Maintain patient confidentiality and protect patient information in accordance with HIPAA policies.
- Work collaboratively with patients, providers, staff, vendors, community partners, and visitors in a manner consistent with WCC’s mission and values.
- Be available to work occasional evenings and weekends as needed.
- Perform additional duties as assigned in support of WCC’s mission.
