Purpose:   The Community & Behavioral Services (CBS) Case Manager, non-licensed   provides comprehensive, culturally responsive case management services to   Tribal citizens and other participants receiving services through the CBS   Healing Center. The position serves as a central point of coordination among   participants, families, Healing Center providers, Tlingit & Haida   departments and programs, community organizations, and external service   providers.

The Case Manager partners with participants to   identify needs and work toward the goals established in their person-centered   service or treatment plans. Depending on the participant’s needs, this may   include conducting additional needs assessments, developing and coordinating   individualized service plans, or supporting implementation of   clinician-developed treatment plans. Responsibilities include connecting   participants with appropriate resources, facilitating referrals, advocating   for access to services, monitoring progress, maintaining accurate and   confidential records, and coordinating communication across providers and   systems.

The Case Manager helps participants navigate   behavioral health, medical, housing, education, employment, transportation,   family, social service, and other systems that may affect their wellness,   stability, and ability to achieve their identified goals. Services are   provided using strengths-based, trauma-informed, culturally responsive, and   participant-centered practices that honor the importance of Tribal identity,   family, community, culture, traditional practices, and self-determination in   healing and wellness.

 

Essential Functions. An individual in   this role must be able to perform the following functions with or without reasonable accommodation:

· Receive and respond to referrals from CBS Healing   Center providers, Tribal programs, community partners, and other authorized   referral sources.

· Conduct participant intake and gather information   within the scope of the position.

· Identify participant needs, barriers, resources, and   service priorities.

· Assist participants in identifying short- and   long-term goals and developing individualized case management plans.

· Obtain required releases of information and other   consent documentation in accordance with applicable requirements.

· Reassess participant needs and goals as   circumstances change.

· Provide ongoing case management and follow-up to   promote continuity of care and progress toward identified goals.

· Develop and maintain current knowledge of Tribal,   state, federal, nonprofit, healthcare, behavioral health, social service, and   community resources.

· Make timely and appropriate referrals based on   participant needs and eligibility.

· Assist participants with navigating referral   processes, applications, appointments, eligibility requirements, and other   access barriers.

· Coordinate communication between participants and   service providers when authorized.

· Follow up on referrals to determine whether services   were successfully accessed.

· Serve as a liaison between the CBS Healing Center   and Tlingit & Haida departments and programs.

· Coordinate with Tribal and external partners to   address participant needs and facilitate continuity of services.

· Participate in interdisciplinary and interagency   case consultations and staff meetings as appropriate.

· Maintain professional relationships with partner   organizations and community service providers.

· Develop familiarity with referral criteria,   eligibility requirements, contact procedures, and service limitations for   commonly used resources.

· Elevate unresolved service barriers through   appropriate supervisory and organizational channels.

· Work collaboratively with CBS Healing Center   clinicians, contractors, and other providers to support coordinated   participant services.

· Support continuity between behavioral health   services and other social, medical, community, and Tribal resources.

· Participate in case staffing and interdisciplinary   consultation when requested.

· Support wellness activities and services that   promote long-term stability, resilience, and connection to community and   culture.

· Maintain complete, accurate, timely, and organized   participant records within the designated electronic record-keeping system.

· Document intake, referrals, contacts, service   coordination, case staffing, follow-up activities, and case outcomes.

· Protect confidential participant information and   maintain records in accordance with applicable privacy and confidentiality   requirements.

· Complete documentation within established timelines.

· Ensure records accurately reflect services provided   and participant progress.

· Other duties as assigned

Minimum   Qualifications (education, experience, skills):

· Associate   degree in Social Work, Human Services, Behavioral Health, Public Health,   Psychology, or another closely related field OR an equivalent combination of   education and relevant experience that demonstrates ability to successfully   fulfill position functions.

· Three (3) years of experience in case management,   social services, behavioral health, community services, healthcare   coordination, or a closely related field.

· Must be 21 years of age with a Valid driver's license.

· Ability to   successfully complete required background checks and other employment requirements   applicable to the position.


Preferred   Qualifications (education, experience, skills):

· Bachelor's degree in Social Work, Public Health,   Human Services, Psychology, Behavioral Health, or a related field.

· Experience providing case management services to   Tribal citizens or Alaska Native communities.

· Experience working in behavioral health, social   services, healthcare, family services, or community-based programs.

· Knowledge of Tlingit & Haida culture,   communities, and Tribal programs.

· Experience coordinating services across multiple   agencies or service systems.

· Experience working with electronic health records or   case management information systems.

· Experience with trauma-informed care, motivational   interviewing, strength-based case management, or similar approaches.

· Knowledge of Alaska's behavioral health and social   service systems.
