The Case Manager III provides supportive case management and care coordination services to individuals. Working under the guidance of the Program Manager and alongside an interdisciplinary team, the Case Manager III helps patients access behavioral health, substance use disorder (SUD), medical, and community-based services.
This is an excellent opportunity for candidates who are early in their behavioral health careers and are interested in developing skills in case management, care coordination, substance use treatment, and patient engagement. The position utilizes evidence-based approaches including Motivational Interviewing, Strengths-Based Case Management, and ASAM-informed care.
The Case Manager III is expected to demonstrate empathy, professionalism, strong organizational skills, cultural humility, and a commitment to supporting patients on their recovery journey.
Essential Duties and Responsibilities
Clinical Services & Patient Support
- Assist with patient intake, screenings, and basic needs assessments.
- Support the completion of ASAM-informed screening and referral processes under clinical supervision, as appropriate.
- Help patients identify goals related to their health, recovery, and overall well-being.
- Utilize Motivational Interviewing and strengths-based approaches to engage and support patients.
- Coordinate referrals to behavioral health, substance use treatment, medical providers, housing resources, and community services.
- Conduct outreach and follow-up to ensure patients successfully connect with recommended services.
- Provide warm handoffs and assist with closed-loop referral processes.
- Support patients during challenging situations and notify supervisors of crisis concerns according to program protocols.
- Maintain accurate and timely documentation in electronic health records and other required systems.
- Communicate patient progress and care coordination activities with team members.
Care Coordination & Team Collaboration
- Participate in multidisciplinary team meetings and case review discussions.
- Collaborate with internal programs including Admissions, Residential Services, Outpatient Services, Opioid Treatment Program (OTP), Perinatal Services, and other programs as needed.
- Work collaboratively with community partners, healthcare providers, probation departments, and social service agencies.
- Assist in outreach and engagement efforts for patients referred by internal and external partners.
- Support integrated care efforts across substance use, behavioral health, and primary care services.
Administrative Responsibilities
- Complete required documentation, reports, and data tracking activities.
- Attend required trainings, team meetings, and supervision sessions.
- Maintain compliance with HIPAA, 42 CFR Part 2, and organizational policies.
- Perform other duties as assigned.
- Bachelor's degree in Social Work, Psychology, Human Services, Behavioral Health, Public Health, Sociology, or a related field; OR
- Associate degree with relevant internship, volunteer, peer support, or direct service experience.
- Strong interpersonal, communication, and organizational skills.
- Ability to work effectively with diverse populations.
- Basic computer and documentation skills.
- Commitment to recovery-oriented and person-centered care.
- Valid California driver's license and acceptable driving record.
Preferred
- Experience working with individuals affected by substance use disorders, mental health challenges, homelessness, or justice system involvement.
- Registration or certification through CADTP or CCAPP.
- Experience as a Peer Support Specialist, Behavioral Health Technician, Community Health Worker, or related role.
- Knowledge of Motivational Interviewing and strengths-based practices.
- Bilingual English/Spanish skills.
- Familiarity with HIPAA and 42 CFR Part 2 confidentiality requirements.
