The Family Healthy Advocate- Father Involvement engages fathers, father figures, and families in father involvement services, including outreach, case management, care coordination, fatherhood initiatives, and family life course education. This position conducts needs assessments and provides ongoing case management, care coordination, tracking, and referrals in accordance with established Camden Healthy Start (CHS) guidelines, policies, and procedures. The Father Involvement Case Manager supports pregnant and parenting women, as well as fathers and father figures enrolled in the program, with a primary focus on engaging, supporting, and advocating for fathers. This individual maintains collaborative relationships with community and healthcare providers to facilitate access to health, social, and support services, promote family well-being, and improve outcomes for parents, children, and families. The position requires experience in outreach, case management, care coordination, and working with individuals and families, with a strong emphasis on engaging men and fathers in services that promote healthy families and positive outcomes. 


Essential Functions:

*Contribute to The Cooperative’s commitment to equity and continuous quality improvement that aligns with the agency’s mission, goals, and advancement. *Effectively cross-collaborate with agency staff, leaders, and board members to foster a comprehensive approach of information sharing, working committees, and collective representation. *Routinely disseminate relevant information, presentations, training, and learning opportunities for internal program and agency staff. *When applicable, will participate in relevant staff development opportunities (trainings, summits, conferences, etc.) to increase skill development, elevate program goals, and engage staff members with timely key learnings. *Works in conjunction with and as part of the Family Health Advocate care coordination team to engage fathers, father figures, partners, and spouses of pregnant and parenting women in CHS services and father involvement programming. *Maintains an active caseload of pregnant and parenting women, fathers, and father figures enrolled in the program. *Conducts participant outreach, intake, needs assessments, screening, and data collection using approved program tools and established CHS guidelines, policies, and procedures. *Develops, implements, and maintains individualized service plans with participant input regarding goals, priorities, and identified needs. *Provide and documents case management, care coordination, education, referral, and follow-up services in compliance with program requirements. *Identifies resources and links participants to needed services, including healthcare, behavioral health, mental health, workforce development, parenting support, social services, and other community-based resources. *Implements father-focused educational programming, including but not limited to 24/7 Dad, Responsible Fatherhood Curriculum, and The Nurturing Father's Program. *Ensures participant engagement and retention through ongoing follow-up, educational support, care coordination, and referrals. *Maintains ongoing relationships with community organizations, healthcare providers, social service agencies, and other stakeholders to facilitate access to services and improve outcomes for families. *Communicates and collaborates with referring providers, care coordinators, case managers, and other professionals involved in participant care, as appropriate. *Complete all required documentation, data entry, reports, screening tools, and program records accurately and in a timely manner. *Participate in case conferences, care coordination meetings, supervisory meetings, quality improvement activities, training sessions, coalition meetings, and other meetings as assigned. *Participates in special events, including but not limited to baby showers, health fairs, speaking engagements, focus groups, community outreach activities, and family engagement events. *Advocates on behalf of participants and assists in addressing barriers to care, services, and community resources. *Conducts networking and outreach activities to recruit participants and strengthen partnerships with community stakeholders. *Serves as a spokesperson for CHS father involvement programming at professional meetings, community events, and with media representatives, as appropriate. *Maintains participant and staff confidentiality in accordance with organizational policies, procedures, and applicable regulations.

Requirements
  • Associate's degree required; Bachelor's degree preferred
  • 2 years’ experience in outreach and case management or providing direct services to clients, including working with men in social services, fatherhood programming, community, or correctional settings
  • Excellent interpersonal skills to related to men, women, providers, community, and professionals 
  • Basic computer skills necessary
  • Must have transportation and valid driver’s license/insurance
  • Ability to work at least 2 evenings and 1 weekend day a month
  • Knowledge of community resources
  • Strong organizational skills; ability to prioritize work requirements
  • Ability to work well with consumers and providers
  • Experience with maternal and child health issues and target population a plus

  • Must be able to operate a motor vehicle
  • Must be able to lift up to 10 pounds
