POSITION TITLE: Diabetes Educator

CLASSIFICATION:  Exempt 

DEPARTMENT:  Tribal Health

SUPERVISOR: Diabetes/ Wellness Manager or Designee

GRADE: E14 $27.72 ($57,662.59) - $41.58 ($86,493.89)

Position Summary:

Under direct supervision of the Diabetes/ Wellness Manager is responsible for planning, organizing and directing program and activities dealing with the promotion of health through the application of evidence-based scientific knowledge of diabetes related education and clinical nutrition. Will be responsible for providing diabetes related education and nutrition counseling to those identified with Type 1 and Type 2 diabetes, gestational diabetes, patients diagnosed with pre diabetes and those with family histories. Will function as a collaborative member of the Diabetes Team for routine care & contributing to on-going health and wellness activities.

Essential Duties & Responsibilities:

· Plans, organizes, and directs education and nutrition services for the Health Center

· Provides the nutrition support for the Health Care Team including Community Health

· Identifies, develops and distributes diabetes related and nutritional education material for the Health Center and all communities.

· Provides diabetes and nutritional education to meet prevention and treatment needs for individuals as well as specific groups (not limited to) pre-natal, youth, diabetic, pre diabetic, obese

· Provides alternative food preparation methods to specific groups – Commodity Food Program and others upon request

· Plans for and administers the Summer Foods Program in accordance with SFSP regulations.

· Assists Senior Program staff in menu development; certifies menu.

· Provides referred clients with an evaluation and objective based nutritional care plan i.e. individual counseling

· Assists in developing budget for the diabetes education and nutrition program; performs related work as required.

· Contributes to monthly, quarterly, and annual reports.

· Performs other duties as assigned.

Requirements

Minimum Qualifications:

· Certified Diabetes Educator (CDE) preferred

· Master’s degree (preferred) or bachelor’s degree in nursing or nutrition from an accredited college or university.

· Experience in clinical and community nutrition strongly preferred

· Clinical experience working with people with diabetes, pre diabetes, cardiovascular disease, obesity, etc. required

· Demonstrated ability to communicate and work effectively with Native Americans. Must be able to work in a diverse minority environment

Knowledge, Skills and Abilities

· Ability to communicate effectively, both verbally and in writing.

· Ability to represent the organization in a professional manner, building respect and confidence.

· Ability to handle multiple tasks and meet deadlines.

· Ability to carry out instructions furnished in verbal or written format.

· Ability to work independently with minimal supervision.

· Skill in operating business computers and office machines, including Windows (i.e. Word, Excel, PowerPoint etc.)

Physical Demands: 

While performing the duties of this job, the employee regularly is required to sit, use hands to finger, handle, or feel; reach with hands and arms, and talk to hear. The employee frequently is required to walk, the employee occasionally is required to operate a motor vehicle; stand, stoop, squat, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 45 pounds. 

Work Environment:

Work is generally performed in an office setting within a health and wellness facility with moderate noise level. Travel is required for training, workshops, and conferences. Evening and weekend meetings are common.
