Description

The Social Services Designee provides and coordinates psychosocial services for residents in Long-Term Care and Assisted Living. The position supports residents and families throughout admission, residency, care planning, transitions, and discharge while promoting resident rights, dignity, choice, independence, and quality of life. The Social Services Designee collaborates with the interdisciplinary team and reports directly to the Executive Director.

  

· Complete required psychosocial assessments, progress notes, care plan documentation, and other social services records within required timeframes.

· Participate in admissions, care conferences, interdisciplinary meetings, discharge planning, and resident transitions between levels of care.

· Provide support to residents and families related to adjustment, changes in condition, grief/loss, family concerns, placement, and other psychosocial needs.

· Advocate for resident rights, preferences, privacy, dignity, self-determination, and access to the Long-Term Care Ombudsman or other resources.

· Assist with resolution and documentation of resident/family grievances, interpersonal conflicts, missing belongings, and other psychosocial concerns.

· Assist with behavioral and psychosocial care planning, including dementia-related behaviors, anxiety, depression, adjustment concerns, and mental health referrals.

· Provide information regarding advance directives, healthcare power of attorney, guardianship, decision-making arrangements, and end-of-life resources as appropriate.

· Assist residents and families with education regarding facility policies, resident rights, available services, and transitions to another level of care.

· Support Resident Council and Family Council activities and follow up on concerns or requests assigned to Social Services.

· Collaborate with Nursing, Therapy, Admissions, Activities, Dietary, Business Office, and other departments to address resident needs and continuity of care.

· Participate in investigations and abuse/neglect reporting processes when assigned, in accordance with facility policy and applicable reporting requirements.

· For Assisted Living residents, participate in service planning, psychosocial support, family communication, and transition planning on a as needed basis.

· Assist with the STR unit as needed.

· Maintain confidentiality and comply with HIPAA, CMS, Ohio Department of Health, resident rights, and facility policies and procedures.

Requirements
  • High School Diploma or equivalent required, degree in social work, human services, psychology, sociology, or related field preferred
  • Strong communication skills, organizational skills, documentation, problem-solving, conflict resolution, and confidentiality required
  • Knowledge of resident rights, person-centered care, discharge planning, and psychosocial needs of older adults preferred
  • Experience in long-term care, assisted living, healthcare, case management, or social services preferred
