Summary
The Hospice Admissions Nurse performs hospice consultation and admission visits to assess patient and family needs, collaborate with the attending physician and the hospice physician, and help determine the patient’s clinical eligibility for hospice care; completes the admission assessment and Hospice Election of Benefit documentation for Peoples Hospice; serves as a resource, provides community outreach and education to physicians, facility staff, clients, and caregivers regarding hospice services; professionally and comprehensively presents the Company’s hospice services as necessary to educate and inform medical community.
Responsibilities
- Performs consultation and/or admission visit to assess patient/family needs and collaborates with the patient’s attending physician and the Hospice physician to determine the patient’s clinical eligibility for hospice care.
- Completes admission assessment and Hospice Election of Benefit documentation for Peoples Hospice.
- Demonstrates a high degree of responsibility for attendance and uses work time in a productive and efficient manner. Working independently most of the time requires excellent time/territory management skills
- Participates in Company functions to build community and public awareness of Peoples Hospice’s services
- Maintains accurate records, documents actions, maintains control of budgeted expenses and submits required reports on time
- Collaborates with the patient’s attending physician and the Hospice physician to assist him/her in determining the patient’s clinical eligibility for hospice care
- Assesses patient’s medical status, planning, teaching and implementing immediate medical interventions on day of admission, as prescribed by physician’s orders
- Actively participates in coordinating all aspects of patient’s care, according to current professional standards and practice
- Participates in ongoing interdisciplinary comprehensive assessment, as needed
- Manages a caseload of four to five patients
- Develops the initial Plan of Care (POC) and updated POC, as needed, in collaboration with the interdisciplinary group (IDG), the patient’s attending physician, the Hospice physician, and the patient/family
- Coordinates medications and durable medical equipment (DME) needs for same-day delivery
- Participates in after hours and weekend on call rotation
- Responsible for completion of all elements of admission process and first 24-hours of care
- Performs timely administrative functions relating to admissions, clinical records, Plan of Care, and mandatory abuse reporting requirements
- Communicates admissions information, issues and concerns in patient care meetings to assure smooth transition and continuity of care with other IDG staff, including a detailed report to the assigned Case Manager and other assigned disciplines
- Provides consultation and emotional/physical support to patients/families regarding Hospice services and disease processes
- Performs routine nursing visits when requested
- Accept other assignments as appropriate
Required Qualifications:
- Current Florida or multi-state license required.
- Two years of RN experience is preferred.
- Experience in acute, oncology, home health, and/or hospice setting preferred
- Availability to work evenings, nights, weekends, and holidays.
- Current CPR certification is required.
- Valid driver’s license and current auto insurance is required.

Work Environment
While performing the duties of this position, the employee travels by automobile and is exposed to changing weather conditions. The employee works in varied residential environments based on assigned caseload.

While performing the duties of this job, the employee is required to see, talk, or hear. The employee is regularly required to stand, walk, sit, reposition patients; use hands and fingers to feel objects, tools, or controls; and reach with hands and arms. The employee frequently lifts and moves up to 50 lbs.
