The Hospice Nurse Practitioner (NP) delivers compassionate, patient-centered care to individuals with terminal illnesses. This role involves performing face-to-face visits to assess patient eligibility for hospice services, recertify ongoing care, manage symptoms, and coordinate with interdisciplinary teams to ensure high-quality, compliant care. The NP serves as a clinical resource for patients, families, and hospice staff, providing guidance on symptom management, disease trajectories, and end-of-life planning.
Essential Duties:

- Conduct face-to-face assessments for hospice eligibility and recertification requirements.
- Develop and implement individualized care plans addressing physical, emotional, and spiritual needs.
- Manage pain and symptoms, including prescribing medications and adjusting treatments as needed.
- Collaborate with physicians, nurses, social workers, chaplains, and other healthcare professionals to provide comprehensive care.
- Provide emotional support and education to patients and families regarding disease progression, prognosis, and end-of-life decisions.
- Document patient care accurately in electronic medical records and ensure compliance with hospice regulations and Medicare standards.
- Serve as a clinical resource for staff and the community regarding hospice eligibility, levels of care, and symptom management.
- Travel to patient homes, hospice facilities, or long-term care centers as required.
- Other duties as assigned.
Qualifications, Education and/or Experience:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and abilities required.
- Master’s degree in Nursing and completion of an accredited Nurse Practitioner program.
- Current RN and NP licensure in the state of practice; national NP certification (ANCC or AANP).
- Minimum of 1–2 years of clinical experience, preferably in hospice, palliative care, geriatrics, oncology, or home health.
- Knowledge of end-of-life care, pain management, and palliative care principles.
- Strong communication, interpersonal, and emotional resilience skills to support patients and families.
- Ability to use electronic medical records and maintain compliance with healthcare regulations.
- CPR certification and, if applicable, controlled substance agreement

- Regular travel is required to perform face-to-face patient visits throughout the service area.

- While performing the duties of this job, the employee is occasionally required to stand for extended periods of time, move, communicate, and identify written information. The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl and lift and/or move up to fifty (50) pounds independently, and occasionally lift and/or move up to two hundred (200) pounds with assistance.
- Work for this position is performed in patient homes, care settings/facilities, and office settings. This position will have to navigate through a variety of different settings to provide care.
- There is exposure to varied home environments and potential infectious conditions.
