Schedule: Monday-Friday
Two Full-time openings: 40 Hours to fit the needs of the program
One Part-time opening: 20 Hours to fit the needs of the program
Pay: $23 per hour – Paid training provided by Venture!
Sign On Bonus: $1000 for Full Time and $500 for Part Time employees
POSITION SUMMARY
The Supports Case Worker reports to the Individual Supports Coordinator. This position provides person-centered case management, coaching, and coordination to individuals served through Supported Collaborative Living (SCL) and Independent Living Supports (ILS). The Case Worker assists individuals in maintaining the greatest possible level of independence in their homes and communities while helping ensure that supports, services, protocols, and required documentation are in place and followed.
ESSENTIAL DUTIES AND RESPONSIBILITIES
· Build respectful, professional relationships with individuals served and provide support that reflects each person's strengths, choices, goals, and right to independence.
· Assist individuals in maintaining an independent living situation, including developing routines and practical skills needed to live safely and successfully in the community.
· Provide coaching and support with budgeting, bill payment, banking, financial responsibilities, and other money-management systems, while encouraging informed decision-making and independence.
· Assist individuals in maintaining a safe, sanitary, and organized apartment or home, including attention to household routines, supplies, food, and cleanliness.
· Help individuals access appropriate medical, dental, behavioral health, and other needed care; assist with appointment scheduling, follow-up, communication with providers, and implementation of recommendations as authorized.
· Provide oversight to ensure that each individual has an appropriate system for taking medications in accordance with the individual's plan, applicable law, delegated authority, and agency policy.
· Ensure that individual service plans, protocols, risk-management documents, releases, emergency information, and other required records are current, in place, and implemented as directed.
· Document services, progress, contacts, concerns, and follow-up actions accurately and on time in accordance with agency requirements and applicable regulations.
· Assist individuals with transportation planning and access to community resources while teaching and reinforcing skills that increase independence in travel and community participation.
· Participate in person-centered planning, interdisciplinary team meetings, staff meetings, reviews, and other meetings required to coordinate effective supports.
· Communicate and collaborate with individuals, families, guardians, service providers, and members of the support team, consistent with applicable consents, confidentiality requirements, and agency policy.
· Recognize and promptly report health, safety, environmental, medication, or service concerns; follow required incident-reporting, abuse/neglect reporting, and emergency procedures.
· Attend required agency training, maintain required certifications, and follow all policies, procedures, and standards applicable to the position.
MARGINAL DUTIES AND RESPONSIBILITIES
· Maintain an administrative office presence at least once each week, or as directed by the supervisor.
· Attend administrative, staff, and supervisory meetings as required.
· Offer recommendations to the supervisor regarding ways to improve systems, services, and supports.
· Maintain confidentiality, professional boundaries, accountability, and a positive demeanor while managing multiple priorities.
· Maintain current CPR and First Aid certifications, as required by agency policy.
· Perform other duties as assigned.
PHYSICAL AND MENTAL REQUIREMENTS
· Effective oral and written communication skills.
· Ability to establish and maintain professional rapport with individuals with developmental disabilities, their families, and service providers.
· Strong time-management, organization, documentation, and problem-solving skills.
· Ability to make and keep appointments, manage a flexible schedule, and work independently in community settings.
· Ability to plan transportation and travel as needed to individual residences, appointments, meetings, and community locations.
· Ability to remain calm, respectful, and responsive while addressing changing needs and multiple priorities.
· A bachelor's degree in social work, human services, psychology, rehabilitation, or a related field is preferred; however, a bachelor's degree is not a minimum requirement for this position.
· High school diploma or GED required.
· Minimum age of 21 years.
· Valid driver's license, acceptable driving record, access to reliable transportation, and availability by telephone for routine work-related communication.
· Experience in direct support, case management or independent-living support is preferred.
· Ability to communicate effectively, verbally and in writing, in English, consistent with the essential functions of the position.
