St. Jude Neighborhood Health Centers is seeking a full-time LCSW or LMFT to engage in outreach, assessment, and treatment of the needs of unhoused persons living on the streets of north Orange County. Outreach will be undertaken in partnership with The Hope Center of Orange County and will primary focus on servicing unhoused women age 40 and older. Most of this job will involve performing work outdoors and interacting with individuals living on the streets and will be undertaken as part of a mobile team.  This position if funded via a one-year grant, with potential to extend the grant for additional year.

Essential Functions (What You Do)

· Complete psychosocial evaluations, assessments, and provide counseling services to a diverse population of patients with a wide range of mental health diagnosis, including individuals with a history of substance use disorder. 

· Provide effective follow-up services and community referrals as appropriate.

· Provide crisis intervention to patients.

· Initiate and complete all state-mandated documentation within specified time frames on child and elder abuse and domestic violence.

· Collaborate with medical providers, nurses, patients to assess patients’ social needs and monitor progress of treatment.

· Ensure timely documentation of all patient contact in clear, concise and accurate notes with the patient’s electronic health record.

· Maintains a caseload; performs a variety of direct clinical, diagnostic, treatment, and outreach services.

Requirements

Education:
Master’s Degree in Social Work or Marriage and Family Therapy from an accredited University. 


License/ Certifications:

· Current and valid license in good standing – Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT) through the California Board of Behavioral Sciences.  

· Provider level basic life support (BLS) from AHA approved program. 

 
Experience:
Two years' recent experience, preferably working unhoused persons. 


Knowledge/Skills/Abilities: 

  • Office administrative practices and procedures, including EMR (preferably EPIC), management and the operation of standard office equipment. 
  • Familiar with the culture and customs of the clients with whom you will interact.  
  • Communication skills consistent with the facilitation of mental health wellness topics to individuals, small and large groups of community residents.  
  • Effective written and verbal communication across all levels of the organization and with vendors.
  • Highly organized with strong time management skills, being able to perform simultaneous tasks, prioritize them effectively and follow through quickly. 
  • Proficiency with computer software applications (e.g., Microsoft Word, Excel, Outlook, PowerPoint; Internet) 

Preferred Requirements

Bilingual in English and Spanish, is preferred but not required.

Experience working with people experiencing homelessness

Experience providing patient care in non-traditional environments (i.e. streets and /or shelters)

Salary Description
$90,775.00 to $111,354.00 per year
