** Case Manager 1 provides community-based case management and care coordination services for Veterans,

service members, and their families participating in the Staff Sergeant Parker Gordon Fox Suicide Prevention

Grant Program (SSG Fox SPGP).

This role supports individuals who may be at risk for suicide by conducting needs-based and program

eligibility assessments, coordinating services, and connecting participants to VA and community resources

that address behavioral health needs and social determinants of health.

The Case Manager engages in outreach, develops individualized service plans, and promotes participant

stability, recovery, and overall well-being using trauma-informed and recovery-oriented practices. The Case

Manager 1 is charged with making demonstrable progress toward applying field-relevant best practices and

suicide prevention strategies.


 

ESSENTIAL DUTIES AND RESPONSIBILITIES

· Ensures that the Veterans’ voice is heard, and when appropriate the Veterans’ ideas play a prominent role

in programming.

· Provides case management and care coordination services to Veterans, service members, and their

families enrolled in the program.

· Conducts outreach and engagement to identify eligible Veterans not currently connected to VA or

behavioral health services.

· Conducts needs-based screenings and assessments, including required baseline mental health and suicide

risk screenings and identifies appropriate services.

· Develops and implements individualized service plans that address suicide risk, stability, and long-term

well-being.

· Coordinates services with VA Medical Centers, Vet Centers, and community-based providers.

· Supports participants in accessing behavioral health, housing, benefits, and other stabilization resources.

· Responsible for understanding and implementing their assigned program according to program funder

and Volunteers of America standards and expectations.

· Conducts community-wide outreach to identify eligible participant households presenting with substantial

barriers to housing stability.

· Supports team members by serving as a resource for analyzing and solving problems and staying abreast

of current issues and theories within the field.

· Attends orientations, trainings, education programs, staff meetings, community meetings, conferences

and workshops as requested and applicable to meet the needs of the position

· May provide training and mentorship to team members and community stakeholders regarding best

practices in relevant service models and practices.

· Clearly documents all client interactions along with required eligibility and demographic information

 · Performs duties in a professional manner by maintaining the confidentially of all information and by participating effectively within and across teams. 

· Participates in professional development activities to promote the development of knowledge and experience in field-relevant best practices and standards and makes demonstrable progress towards working in accordance with these practices and standards. 

· Performs all other duties as assigned.

WORKING CONDITIONS AND PHYSICAL REQUIREMENTS 

· Travel throughout the program’s service area is required on a regular basis. 

· Must be able to work in diverse and, at times, uncomfortable environments such as homeless shelters, service facilities, streets, offices, and all other locations as necessary to fulfill program objectives. 


POSITION TYPE AND EXPECTED HOURS OF WORK 

· Full-Time or Part-Time as indicated in posting.

· Work hours may vary but are typically scheduled around a 40-hour workweek designed to fulfill program objectives with occasional overtime requirements. 


 LOCATION · Western Colorado Veteran Services Office 

PAY RANGE:  $22.00-$24.00/hr

Requirements

  COMPETENCIES

 MINIMUM QUALIFICATIONS OF POSITION

 · Bachelor’s degree in human services, social work, or a closely related field or related experience. 

· Must possess a Colorado driver’s license and state-mandated automobile insurance. 

· Must possess a personal vehicle that may be used for work-related travel (reimbursement for mileage is available). 

· Must complete agency and program credentialing within 12 weeks of hire and maintain credentialing standards thereafter. 


PREFERRED QUALIFICATIONS OF POSITION 

· SOAR (SSI/SSDI Outreach, Access, and Recovery) Certification

 · Experience working with individuals experiencing behavioral health challenges, trauma, or housing instability preferred. 


KNOWLEDGE AND SKILLS 

· Knowledge and skill in the application of Critical Time Intervention, Motivational Interviewing, Trauma Informed Care Principles.

 · Ability to respectfully and professionally serve individuals hailing from diverse backgrounds, cultures, ideologies, and religions.
